Description
RFQ Quote Spreadsheet Terms, Conditions and Instructions Product Specifications Solicitation Questions and Answers This solicitation is amended to make the following changes: 1)Include a copy of all questions asked by interested quoters and provide answers. 2)Quoters are required to submit a separate signed affirmative statement that their flags adhere to the Buy-American Act and adhere to all the requirements within the solicitation pertaining to materials being made, assembled, and sourced within the United States. 3)The delivery schedule has been modified within the "RFQ Quote Spreadsheet." Quoters are notified that the only fields that shall be altered in the spreadsheet are the Vendor Name, DUNS #, Unit Price and the Dollar Value per CLIN. All other fields shall remain untouched and unaltered. 4)Quoters are reminded that the sample flags must meet all of the product specifications and be a true representation of what will actually be provided in the purchase order. 5)Quoters are reminded that the following items must be submitted to constitute a complete quote (extra emphasis on bold and underlined text) : a. Completed Request for Quote spreadsheet. b.Quoter shall review FAR Provision 52.212-3 and complete paragraph (b) or (c) through (r). The completed provision shall be submitted with the quote. c. A separate, signed, affirmative statement that the proposed flags adhere to the Buy-American Act and adhere to all the requirements within the solicitation that state all flag components shall be sourced, manufactured and assembled in the United States, using only items made/produced in the United States, such as the wood, or plastic composite, for the staff and ball top and fabric. d. Two sample flags (that meet all of the product specifications) to be sent, and received by close of Solicitation, to: i.1-flag sent to: 70 Kimball Ave, South Burlington, VT 05403, c/o Steve Snider ii.1-flag sent to: DHS, USCIS, Management Directorate, 633 3rd Street, 2nd Floor, Suite 250, Washington, DC 20529, c/o Angela Owens e.Technical: A technical volume, outlining the quoter's technical capability to meet the requirements outlined in the Product Specifications. The technical volume shall not exceed _5_ pages. A technical volume that addresses this paragraph and the items directly below is required. A simple affirmative statement that the provided product will meet all specifications of the RFQ will not suffice and will not be considered technically acceptable. i. It shall include a detailed description addressing the ability to meet the "Specific Requirements/Tasks" / Delivery Schedule. ii.DELIVERABLES: (ie. Certifications, Warranties, Spec Sheets) iii. List of any/all subcontractors and/or suppliers. f.Past Performance: (maximum of _3__ pages) (non-submission of past performance results in a "neutral" rating for past performance which provides neither an advantage or a disadvantage to the quoter) i.Provide a list of customer contact information for not less than three (3) but not more than five (5) projects performed within the past three (3) years. Present the following information: 1.Customer Name and Address 2.Contract Number/Contract Title 3.Delivery Schedule/Period of Performance 4.Contract Value 5.Description of work performed and how it impacts your ability to meet the requirements of the solicitation 6.Customer point of contact (name, telephone number and e-mail address) ------------------------------------------------------------------------------------ This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. U.S. Citizenship and Immigration Service is issuing a solicitation for up to 4,800,000 customized American Stick Flags. This Request for Quote (RFQ) is for a 12-month period of performance with optional line items. Flag requirements are included in the attached Product Specifications. Flag components shall be sourced, manufactured and assembled in the United States. Two (2) sample flags are required as part of the RFQ submittal process. Questions shall be submitted by 11:00am Eastern Time (ET) on Thursday June 8, 2017. Responses to questions will be disseminated by Thursday June 15, 2017. Quotes are to be received no later than: Thursday, June 22, 2017 at 11:00am Eastern Time. Attached you will find: • Product Specification • Terms and Conditions • Request for Quote Spreadsheet Instructions: 1) On the Request for Quote spreadsheet; fill in the yellow boxes including: Vender Name, DUNS #, Unit Price, Dollar Value per CLIN and Total Dollar Value. 2) Return to Steve Snider, Sr. Contract Specialist, at: Stephen.L.Snider@uscis.dhs.gov by Thursday, June 22, 2017 at 11:00am eastern time. 3) Copy Jason Hawkins, Contracting Officer, at: Jason.S.Hawkins@uscis.dhs.gov with your RFQ submittal.
