- Terms and Conditions and other FAR and IHS clauses Schedule of Services for RMPS GUI SOW for RPMS Graphical User Interface This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number HHSI2462017Q0028 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. This solicitation is 100% set-aside for small business. The associated NAICS code is 518210 and the small business size standard is $32.5M. A schedule of services is attached. The Statement of Work is attached. Dates and places of delivery and acceptance and FOB point is provided in the solicitation. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition no addenda to the provision is applicable. The provision at 52.212-2, Evaluation-Commercial Items will not be used. All evaluation factors and that will affect the contract award are Past Performance, Technical Experience/Qualifications, Interconnectivity/Network Compatibility and Price. The evaluation factors of Past Performance and Technical Experience/Qualifications, when combined are weighted higher than Interconnectivity/Network Compatibility and cost or price. The Indian Health Service will not consider your quote for award if you fail to respond to the following evaluation criteria. Past Performance: Provide three references that include similar experience in serving similar in scope and final product and demonstrate successful performance under existing and or past jobs. Include name of contractor, contractor contact, current phone number, current e-mail address and dates of service. Technical Experience/Qualifications: Include educational degree(s), programming certification(s), years of experience with Resource Patient Management System (RPMS), and service/scope of all staff anticipated to fulfill contract requirements. Notate who will serve as the primary lead to oversee implementation and coordination of services. Also, describe how contractor will ensure staff maintain experience/qualifications to carryout contract requirements. Interconnectivity/Network Compatibility: Include up to three similar contracts with RPMS interconnectivity/network compatibility requirements to demonstrate successful connectivity under existing and or past jobs. Include name of contractor, contractor contact, current phone number, current e-mail address and connectivity platform utilized. Also, include mitigation strategies that will be implemented during contract term to prevent security breaches. Must be registered in the System for Award Management (SAM) database (reference https://ww.sam.gov Please note that all parties interested in conducting business with the Indian Health Service must be registered in the SAM database. Evaluation Rating Information Five: Outstanding - Quote meets requirements and indicates an exceptional approach and understanding of the requirements. Strengths far outweigh any weaknesses. Significantly exceeds performance or capability standard. Risk of unsuccessful performance is very low. Four: Good - Quote meets requirements and indicates a thorough approach and understanding of the requirements. Exceeded some performance or capability standards. Proposal contains strengths, which outweigh any weaknesses. Risk of unsuccessful performance is low. Three: Acceptable - Quote meets requirements and indicates an adequate approach and understanding of the requirements. Strengths and weaknesses are offsetting or will have little or no impact on contract performance. Risk of unsuccessful performance is no worse than moderate. Two: Marginal - Quote does not clearly meet requirements and has not demonstrated an adequate approach and understanding of the requirements. The quote has one or more weaknesses, which are not offset by the strengths. Risk of unsuccessful performance is high. One: Unacceptable - Quote does not meet requirements and contains one or more deficiencies. Proposal is unawardable. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. Federal Acquisition clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. Additional Clauses or provisions are attached. Federal Acquisition clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition, and is attached.
