Description
This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, and is being conducted under FAR Subpart 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. (ii) The solicitation number is VA251-17-Q-0680 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-69, August 1, 2013. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This is a small business set-aside. The North American Industry Classification System (NAICS) code is 332913, and the size standard is 1000 Employees. (v) This requirement consists of three (3) line items: Requirements: A. Quantity of 650 under sink mounted lead free thermostatic mixing valves which are acceptable for commercial application. Valve body will be of an H pattern and consist of brass construction with 3/8 male quick connect compression fittings, tamper resistant locking nuts and integrated stainless steel mesh strainer. The device must be capable of being set to and maintain a constant discharge temperature of 110 °F. Minimum discharge flow must be no less than 0.5 gallons per minute, (gpm). B. Quantity of 650 low lead content, or lead free check valve with a 3/8 compression check valve assembly consisting of (1) male end and (1) female end and a valve / filter / washer assembly no smaller than 10mm in diameter. External surface shall be composed of either stainless steel grade 304, 316 or a chrome plated brass. C. Quantity of 1300 low lead content or lead free supply lines. The supply line will be 20 in length with 3/8 compression fitting at each end. External material with be composed of braided stainless steel in a grade of either 304 or 316 and internal material with be composed of nylon or other inert material. (vi) To be technically acceptable the products quoted shall be Brand Name or Equal to the items noted above. (vii) Delivery FOB Destination to VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI 48105. (viii) 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, 52.203-98, 52.204-16, 52.204-17 (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. Award will be based on lowest price technically acceptable offer. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via https://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically, the offeror shall complete only paragraphs (c) through (o) of this provision. (xi) 52.212-4 Contract Terms and Conditions - Commercial Items: apply to this solicitation with the following addenda; FAR 52.203-99, 52.232-40, VAAR 852.203-70, 852.232-72, 852.246-71. (xii) 52.212-5 Contracts Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items; The following FAR clauses identified at paragraph b of FAR 52,204-10; 52.212.5 are considered checked and are applicable to this acquisition: 52.204-10, 52.209-6, 52.219-6, 52.219-28, 52.222-3, 52.222-19, 52.222-21; 52.222-26; 52.222-36, 52.222-50, 52.223-18, 52.225-3; 52.225-13, 52.232-34 (xiii) RESPONSES ARE DUE Wednesday, June 14, 2017, by 5:00 p.m. eastern standard time (est).
