- Attachment D - Statement of Work Attachment C - Tax Exemption Attachment B - BAA Attachment A - Rate Schedule RFQ Contractor to provide Professional Healthcare Medical Group, that can fully provide Hematology/Oncology services to patients within the specifications of the requirement, within the standards of practice, and in accordance to The Joint Commission, Centers for Medicare & Medicaid Services (CMS), Health Promotion/Disease Prevention (HPDP) requirements, and PIMC policies. Period of Performance is estimated to start on August 1, 2017 through July 31, 2018. This requirement is being solicited for a base and four (1 year) option years. Contractor shall state in their proposal that the quote shall remain effective for 120 calendar days after close of solicitation. Closing Date is July 10, 2017 at 1700 hours Arizona Time. Deadline for questions is on June 26, 2017, 1700 hours Arizona Time. The Government reserves to right to make a single award, multiple awards or no awards in accordance with provision at FAR 52.215-1 which is included in this solicitation. Contractor is to follow and submit proposals in accordance to Section E Solicitation Provisions and Addendum to FAR 52.215-1 Instructions to Offerors - Competitive Acquisitions.
