- MRI SOW This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The Navajo Area Indian Health Service (IHS) intends to award a fixed-price commercial item contract in response to Request for Proposal (RFP) 2017-KG-0003. This solicitation is issued on an SMALL BUSINESS SET-ASIDE. (iii) The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. (iv) The associated North American Industry Classification System code is 621512 and the small business size standard is $15.0 million. (v) PRICE/COST SCHEDULE - Mobile MRI Services. (vi) PURPOSE OF THE CONTRACT: Task Description: Contractor shall provide Mobile MRI services for the Kayenta Health Center, please see SOW. (vii) Date and Place of Delivery: Date of approved credentialing; Kayenta Health Center, Highway 160, South of Mile Post 394.3, Kayenta, AZ 86033. (viii) PERFORMANCE WORK STATEMENT: The Kayenta Service Unit requires the services of a large bore 1.5T mobile MRI unit on a monthly basis. On occasion, the scanner is required a second day in the month to accommodate for back-log, therefore, a total of 18 days/year is requested, but, is dependent on patient load. See Attached SOW. (ix) Evaluation Factors: The Government reserves the right to issue a single award or multiple awards to the schedule contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following evaluation factors will be considered: 1. Past performance 2. Qualifications of each proposed candidate 3. Technical Capability 4. Logistics 5. Socioeconomic Status These evaluation factors will be weighted more than price. Price will be utilized as a tiebreaker in case of equally acceptable proposals. (x) Copies of: (1) Certificate(s); (2) License; and (3) Insurance. PROVISIONS AND CLAUSES: The following provisions and clauses apply to this acquisition. The FAR provision 52.212-1 Instructions to Offerors Commercial Items; and 52.212-3 Offeror Representations and Certifications Commercial Items are incorporated by reference. CONTRACTING OFFICER AUTHORITY: Authority to negotiate changes in the terms, conditions or amounts cited in this contract is reserved for the Contracting Officer. PRO-CHILDREN ACT OF 1994: The Contractor certifies that it will comply with the provisions of Public Law 103-227, Pro-Children Act of 1994, which imposes restrictions on smoking where federally funded children's services are provided. AWARD DECISION: The Government will award a contract, resulting from this solicitation, to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
