- Sole source determination is made in accordance with (IAW) FAR Part 13.106-1(b), FAR Part 12-Commercial Items and FAR Part 13-Simplified Acquisition Procedures. The service listed below is sole source procurement; therefore consideration of other contractors is precluded for the reasons indicated below. NATURE AND/OR DESCRIPTION OF ACTION BEING APPROVED: This is a justification for firm-fixed price purchase order (no options) to Anderson Brule Architects, Inc. (ABA). A DESCRIPTION OF THE SUPPLIES OR SERVICES REQUIRED TO MEET THE AGENCY S NEED: VAPAHCS seeks continued follow-on engagement with the current Transition Services contractor team to provide expanded space programming services towards fostering a Lean and PACT culture for VA staff at the Monterey Health Care Center (HCC) up to May 31, 2017. IDENTIFICATION OF THE JUSTIFICATION RATIONALE AND IF APPLICABLE, A DEMONSTRATION OF THE PROPOSED CONTRACTOR S UNIQUE QUALIFICATIONS TO PROVIDE THE REQUIRED SUPPLY OR SERVICE. The mission of the Veterans Health Administration (VHA) and VA Palo Alto Health Care Services (VAPAHCS) is the implementation of the Patient Aligned Care Team (PACT) model. The overall goal of the initiative is to transform the VA health care delivery system to one providing more patient-centric care. In the Patient Aligned Care Team, each Veteran works together with health care professionals to plan for whole-person care and life-long health and wellness. The care team considers all aspects of patient health, with an emphasis on prevention and health promotion. Care is coordinated through collaboration. All members of the team have clearly defined roles with a focus on forging trusted personal relationships; the result is coordination of all aspects of health care. PACT uses a team-based approach. The patient is the center of the care team that includes family members, caregivers, and health care professionals a primary care provider, nurse care manager, clinical associate, and administrative clerk. Under IDIQ Contract #VA261-15-D-0312 Anderson Brule Architecture, Inc. was awarded Task Order #VA261-15-J-3138 to perform the initial Monterey Transition Services project to educate and prepare VAPAHCS leadership and Monterey leadership and staff to function effectively in the new PACT environment. Anderson Brule Architecture, Inc. (ABA) is the best positioned based on technical skill, knowledge, familiarity with the project and VA staff, and has been determined to be the only reasonable source available thereby increasing efficiency and reducing potential learning curves. As such, VAPAHCS leadership concurs additional work would be beneficial to the government and has determined it to be essential for ABA to continue engagement with the current Transition Services team to prior to Occupancy in order to successfully accomplish the goals to transition to the new PACT environment. This is not a request for quotes and a solicitation package will not be issued. Sources interested in responding to this requirement must be able to provide compelling evidence that they can provide the abovementioned requirement as specified. A determination by the government not to compete this proposed contractual action based upon responses received is solely within the discretion of the government.
