- Terms and Conditions This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This procurement is being conducted under Simplified Acquisition procedures pursuant to the authority of FAR Part 13.5, Simplified Procedures for Certain Commercial Items; FAR Part 12, Acquisition of Commercial Items (41 U.S.C. 1906 and 1907)). This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Billings Area Indian Health Service (IHS) intends to award a fixed-price commercial item contract in response to Request for Quotation (RFQ)-10-16-004-EB. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-84. The associated North American Industry Classification System code is 561320 and the small business size standard is $27.5 million. 1725 HRS @$______ = $_______. Period of performance: Date of Award to 6/30/18. Work Schedule: 40 hours per week, Mon - Fri, 9:00am to 6:00pm Statement of Work: The pharmacist will provide patient services initially with counseling and basic needs of the patient care; management and daily operation of the outpatient pharmacy processing EHR prescriptions, chronic medication refills, and counseling patient on proper use, precautions and storage of medication. Contractor will be performing a combination of duties in the pharmacy. Pharmacist will provide adequate patient education and counseling, assist with patient monitoring or perform normal duties per required safety standards with compromising patient care. Knowledge required by the position: Keep appropriate records of all dispensing. This shall include controlled drugs. Make appropriate entries into the EHR/RPMS medical record. Individual should be trained in the area RPMS and its relation to EHR programming. Use current pharmacy reference materials to provide accurate and timely drug-related information to members of the medical staff. Participate in all pharmacy based monitoring programs within his/her ability and training. Supervisory Controls: The pharmacist will be under direct supervision of the chief pharmacist, Blackfeet Community Hospital. Schedule may change to additional days as needed. Guidelines: Department will provide the contractor with standard and specialized equipment and supplies needed for the performance and delivery of service including the use of a computer. The department is responsible for getting the contractor access and clearance to all pertinent RPMS/EHR, intranet and computer services necessary to carry out his/her duties. The department must provide the contractor with direction and proper workloads, which are appropriate with normal day-to-day operations in the department. Complexity: Contractor will provide service that will reduce the workload on the department, enhance patient safety, and serve patients in a timely matter. Scope and Effect: Contractor will provide coverage with outpatient work load. Contractor will meet the daily high volume of patient's needs. Personal contacts: Personal contacts are with employees in various organizational elements of the activity, employee's at higher headquarters performing similar services, and employees at supporting installations. Purpose of Contracts: The purpose of contract are made to secure supporting documentation or relay/verify information regarding the existence of any discrepancies; obtain information to process actions and/or information from other sources with reference to unusual problems. Physical Demands: Work is primarily sedentary with some physical exertion required. The work is typically performed in the pharmacy department during high volume of patient care. The pharmacy already operates at a level of staffing that is below the IHS formula for our workload. The pharmacist is to insure the pharmacy is able to provide medication in a safe and timely manner. The nature of the work requires a great deal of intensity and frequently elicits mental and emotional stress. Work Environment: Work is performed in the pharmacy department. Government Furnished Property: The IHS shall provide the contractor with the following: 1.) Office space including a computer, supplies and telephone. Computer security: Pursuant to the Federal Information Security Management Act of 2002 (FISMA), the contractor will be required to complete a course in Computer Security Awareness Training (CSAT). This training must be completed annually by all employees including contractors, volunteers, students and summer externs. The contractor will be required to comply with the Federal Information Processing Standards Publication (FIPS PUB) Number 201, "Personal Identification of Federal Employees and Contractors" and the associated Office Management and Budget (OMB) implementation guidance for personal identity verification for all affected contractor and subcontractor personnel. Contracting Officer Authority: Authority to negotiate changes in terms, conditions or amounts cited in this contract is reserved for the Contracting Officer. Contracting Officer's Representative (COR): The COR shall be responsible for: 1.) Monitoring the contractor's technical progress, including surveillance and assessment of performance and recommending technical changes; 2.) Interpreting the statement of work; 3.) Technical Evaluation; 4.) Technical inspections and acceptance; 5.) Assisting the contractor in the resolution of technical problems encountered during performance of this contract. Invoice Submission: Contractor shall submit invoice to the Billings Area Indian Health Service, Attention: Finance, 2900 Fourth Ave N, Billings, MT 59107 with a copy going to the Supply Department at the Blackfeet Service Unit, 760 Hospital Circle, Browning, MT 59417. Contractor agrees to include the following information each invoice: 1.) Contractor's name and address; 2.) Contact name, title, and telephone number; 3.) Purchase Order Number; 4.) Invoice number and date; 5.) Description, quantity, unit of measure, unit price, and extended price of supplies delivered or services performed; 6.) Remit to address; 7.) Taxpayer id Number or ssi number; 8.) Data Universal numbering system (DUNS) number. Pro-Children Act: The contractor certifies that they will comply with the provision of Public Law 103-227, Pro-Children Act of 1994, which imposes restrictions on smoking where federally funded children's services are provided. Child Care National Agency Check and Investigation (CNACO): A CNACI must be completed for all Indian Health Service (IHS) contract personnel within the Billings Area. Public Law (P.L.) 101-630, Indian Child Protection and Family Violence Prevention Act, requires the IHS to conduct a criminal history background investigation on all contractors performing services in IHS facilities. The character and background investigations will be conducted by the Blackfeet Service Unit. Fingerprints must also be taken as part of the pre-employment process and must be completed before Pharmacist is allowed to work. The fingerprints should be coordinated with the service unit.
