Statement of Work 1 2 Statement of Work VHA PHOENIX Project Chapel Flooring Date Feb 11, 2017 Carl T. Hayden Medical Center 650 E. Indian School Road Phoenix, AZ 85012 POC: Clifford Rambaran Contents: I. Background II. Objectives III. Statement of Work / Requirements IV. Special and Technical Requirements NOTE: No security accreditation package is required and Contractor will not have access to any VA sensitive information. This project does not require acquisition of a service that involves storage, transmitting or exchanging VA sensitive information as this requirement is strictly a construction activity per the SOW below. Background: This project is removal of carpet in the main chapel and two adjoin office and install new Mohawk #483 purple passion or equivalent 2 X2 square tiles Scope of Work / Technical Requirements: Provide all labor and materials to remove existing pews, existing carpeting and quarter round molding in the chapel and two adjoining offices, Prep floor to accept the installation new 2 X2 Mohawk # 483 purple passion or equivalent. Install new quarter round wood molding in chapel and vinyl cove base in the adjoining offices. Contractor shall perform this task in two phases the first is removing pews on half of the chapel, remove carpet install new carpet and reinstall pews. The second phase is removal of pews and carpet of the remaining chapel floor and two offices, install new carpet tiles, pews and quarter round molding around chapel flooring and vinyl cove base in offices The contractor can perform this project during normal duty hours. Project area will be free from all construction debris and dispose in a contractor dumpster. Contractor shall coordinate with COR on phasing of this project so that it will not impact spiritual care for our patients. 3.0 Special and Technical Requirements 1. Contractor will provide the proper PPE during this project, gloves, respirators and eye protection as required. 2. Tack mats will be installed at all entrances and exits of work area. 3. Negative air machines will not be required 4. Containment barriers will be installed to isolate the outside area. 5.
