- 1. This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation that will be issued; quotes are being requested and a written solicitation will not be issued. The Government anticipates a Firm-Fixed Price Purchase Order will be issued from this solicitation. 2. This requirement is being solicited as Request for Quotation (RFQ). The RFQ number is NIH-RFQ-4575391. 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94 and 2005-95, dated January 19, 2017. 4. This requirement is solicited as a 100% small business set-aside. The associated North American Industrial Classification System (NAICS) code for this procurement is 326199. Its associated Small Business Size Standard is 750 employees. 5. List of Item(s): The vendor shall provide the following item(s) in the quantities specified below. 1. Replacement Fabric-Reinforced EPDM Pneuma-Seal for Presray BIO4-X Airtight Door Assembly. Clear Opening Size = 1200wide x 2400high, Part No. BIO4-X-10498-01 or equal----Quantity---8 each 2. Replacement Fabric-Reinforced EPDM Pneuma-Seal for Presray BIO4-X Airtight Door Assembly. Clear Opening Size = 900 wide x 2085 high, Part No. BIO4-X-10498-11 or equal---Quantity-8 each 3. Replacement Fabric-Reinforced EPDM Pneuma-Seal for Presray BIO4-X Airtight Door Assembly Clear Opening Size = 1050 wide x 2150 high, Part No. BIO4-X-10498-12 or equal----Quantity-2 each 4. Replacement Fabric-Reinforced EPDM Pneuma-Seal for Presray BIO4-X Airtight Door Assembly.Clear Opening Size = 1050 wide x 2400 high, Part No. BIO4-X-10498-18 or equal--- Quantity-2 each. 5. Replacement Fabric-Reinforced EPDM Pneuma-Seal for Presray BIO4-X Airtight Door Assembly. Clear Opening Size = 1050 wide x 2150 high, Part No.BIO4-X-10498-29. (RF Door) or equal---- Quantity-2 each. 6. Replacement Wire Mesh EPDM Pneuma-Seal for Presray BIO4-X Airtight Door Assembly.Clear Opening Size = 1050 wide x 2150 high, Part No. BIO4-X-10498-29. (RF Door) or equal ---Quantity---2 each 7. RF Room Silicone Spanner Seal---Quantity-1 each. 6. Quotation Requirements: Quotation must reference: RFQ No. NIH-RFQ-4575391. Vendor's delivery time frame must be specified in their quote. Vendor must quote on each line item listed in this notice to be considered. The items must be quoted in the same order and quantities as listed in this notice. The quote must provide both the single quantity item and total quantity item pricing. Quotations submitted for equivalent items must include technical specifications. 7. Delivery and Acceptance Deliveries required by the contractor shall be made F.O.B. destination to the address/addressee listed below: Delivery Address/Addressee: National Institutes of Health 9000 Rockville Pike Building 13, Platform C Bethesda, Maryland 20892 Unless otherwise specified, deliveries shall be made to the Delivery Point specified above, Monday through Friday (excluding Federal Holidays, between the hours of 8:30 a.m. and 4:30 p.m. in accordance of the delivery location time zone (Eastern Time, Central Time, or Pacific Time). Supplies or services scheduled for delivery on a Federal holiday shall be made the next business day. 52.252-1 Solicitation Provisions Incorporated by Reference (Feb 1998) This RFQ incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. Also, the full text of a solicitation provision may be accessed electronically at this address: www.acquisition.gov/far 8. Provision 52.212-1 Instructions to Offerors - Commercial Items (Jan 2017) is incorporated by reference. 9. Provision 52.212-2 Evaluation - Commercial Items (October 2014) is incorporated by reference. The provision 52.212-2 incorporated above is completed as follows. Quotes received in response to this RFQ will be evaluated using the lowest-priced-technically-acceptable (LPTA) methodology. To be determined "Technically Acceptable" the offeror must meet or exceed all salient characteristics. Award will be made to the vendor whose proposed equipment meets all salient characteristics and whose total price is the lowest quote. 10. Provision 52.212-3, Offeror Representations and Certifications - Commercial Items (Jan 2017) applies to this acquisition. 11. Clause at 52.212-4, Contract Terms and Conditions-Commercial Items (Jan 2017), applies to this acquisition and no addenda to the clause are included. 12. Clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (Jan 2017), applies to this acquisition. 13.
