- SOURCES SOUGHT FOR SMALL BUSINESS VENDORS ESPECIALLY ANY SDVOSB VENDORS. REICHERT ULTRAMATIC PHOROPTOR FOR THE VA HEALTH CARE CENTER, BUTLER, PA. LOOKING TO SEE IF ANY SMALL BUSINESS AND SDVOSB VENDORS WHO CAN FULLFILL THE REQUIREMENTS LISTED BELOW. STATEMENT OF NEED The VA Butler Healthcare System (529) has a need for two (2) Reichert 11625 Ultramatic Phoroptor CYL or equivalent to be delivered and installed at the new Health Care Center (HCC) located at 353 North Duffy road Butler PA 16001. Delivery shall be in May, 2017. 11625, Reichert Ultramatic Phoroptor Cyl, or equivalent Must have lens position and power verified by an auto-indexing, PC-based lensmeter Must have precision geared rotary prism diopters Must have corrosion-resistant materials Must have lens secured by highly precise automated adhesive dispensers Must have precision bearing races Must have minus Cylinder Must have reading rod Must have reading card Must have auxillary Cyl lenser Must have standard commercial warranty Must be mountable to a Reliance 7900 stand 2 EA Installation 1 EA Shipping and Handling 1 EA Trade-in 11625, Reichert Manual Phoroptor SN#39961-02 1 EA 11625, Reichert Manual Phoroptor SN#20978-4 1 EA Both units are currently in service and in useable condition. Trade in cannot occur until October 2017
