Please find attachment for all the information Combined Synopsis/Solicitation FA7000-17-T-0057 AFMOA Medical Equipment Purchases for Offutt AFB This is a combined synopsis/solicitation for commercial medical equipment items, prepared in accordance with (IAW) the format in FAR Subpar 12.6, as supplemented, with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written (formal) solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number, FA7000-17-T-0057, in accordance with Far Parts 12 & 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-95, 13 January 2017. The North American Industry Classification System Code (NAICS) is 339112, Surgical and Medical Instrument Manufacturing and the size standard is 1,000 employees. This acquisition is unrestricted. All responsible sources may submit a quote, which is received timely, will be considered by the 10th Contracting Squadron, USAF Academy (USAFA). The government bares no legal liability for bid cost associated with this request for quote. Vendors should not submit a quote unless they are willing to absorb all cost associated with submitting a quote. Quotes submitted with used, reconditioned, refurbished, gray market or liquidated stock will not be considered for award. Please read and comply with all requirements for submitting a quote under the solicitation instructions and the addenda to FAR 52.212-1. It is the USAF Academy's preference/intent is to acquire only domestic end products per the Buy American Act, except as provided in FAR 25.103. Foreign offers will be evaluated per DFARS 225.502 when comparing domestic and foreign offers. This requirement is for commercial off-the-shelf items, therefore; the manufacturing country of origin will determine if the offer is foreign or domestic. Buy American certification of compliance is required and must be submitted with quote. (See DFARS clause 252.225-7001). 1. DESCRIPTION OF ITEMS: The Salient Characteristics for each are as follows: a. (Rae Systems- MCB3-A3C1REZ-42N) Gas Detection Systems, or Equal Three gas detection systems are required for Offutt AFB  All in one continuous monitoring system with capabilities for gamma radiation, VOCs, oxygen, toxic and combustible gases, for a total of up to six threats at a time. Must be highly customizable with over 25 field-interchangeable intelligent sensor options, with wireless access to real-time instrument readings and alarm status from any location. To include five-way local and remote wireless notification of alarm conditions, including Man down alarm.  Monitors shall have photoionization (PID) sensor with 10-2000 part per billion (ppb) capacity, and 10.6 eV lamp  The monitors shall also have combustible lover explosive limit sensors, carbon monoxide + hydrogen sulfide combo sensors, ammonia sensors, and oxygen sensors  Each monitor will have wireless capabilities (900 MHz built in modem), rechargeable Lithium ion batteries with alkaline adapters E  Each monitor must come with a fully equipped accessory case with quick calibration capabilities.  The unit shall also include a CONF SPACE and CAL kits (4 gas+ 100 PPM isobutylene.  FOB Destination b. (Harris- Item #MEI EMT-1) EMT-L Microscopes, or Equal Eight Scope Body EMT-I LED Microscopes for production of quality oral appliances for Offutt AFB Dental Clinic  Upright and unreversed 3D image  EMT Stereo Microscope has high resolution Meiji optics with 10-20X magnification  Changes from one magnification to another by twisting turret  Arm fully adjustable and allows large working radius  64 bulb LED ring light wired directly into arm, no separate control box  Equipment must be new  FOB Destination c. (Biolase Epic- Part # 7400073) Diode Laser, or Equal Requirement is for a diode laser to be used for gingival surgeries.  Tabletop diode laser with 3 settings for nm output.  940NM wavelength  Disposable, single use fiber optic tips  Options include soft tissue, whitening, and pain therapy.  Our uses could range from frenectomy, to gingivectomy, or crown lengthening.  Equipment must be new  Installation and training  FOB Destination The United States Air Force Academy (USAFA) on behalf of the Air Force Medical Operations Agency (AFMOA), has specific requirement for the purchase of three Brand Name or Equal Requirements, including three (Rae Systems) gas detection systems (or equal), eight (Harris) EMT-L Microscopes (or equal), and one Biolase Epic Diode Laser (or equal). Failure to comply with all instructions contained within this synopsis/solicitation could result in the quote being ineligible for award.
