Description

Notice of Intent to Sole Source Department of Veterans Affairs intends to issue a sole source contract to STERIS Corporation, by other than full and open competition, in accordance with Federal Acquisition Regulation FAR 6.302-1 Only one responsible source and no other supplies or services will satisfy agency requirements to provide maintenance, service, and repair of STERIS sterilizer equipment. This is a special notice only and is NOT a request for competitive quotes or a formal solicitation. The attached document identifies the requirements for any responses by interested parties. Responses may be submitted until 12 pm May 27, 2017 to Paula Moore at paula.moore2@va.gov. Statement of Work Service for Sterile Processing Equipment and Operating Tables DESCRIPTION: The work consists of providing all materials, labor and Equipment necessary for the maintenance, service and repair of the following equipment: Century Gravity Sterilizer (sn: 011690320) with Chamber Cleaning CAVIWAVE Ultrasonic Cleaner (sn: 0210C0788) CAVIWAVE Ultrasonic Cleaner (sn: 0210C0789) PREVAC Sterilizer (sn: 033290801) with Chamber Cleaning RELIANCE Synergy Washer (sn: 3630108003) RELIANCE Synergy Washer (sn: 3630108004) PREVAC Sterilizer (sn: 033310809) with Chamber Cleaning SCS Single Load (sn: 3628808003) SCS Double Load (sn: 3628808004) SCS Single Load (sn: 3628408001) SCS Double Load (sn: 3628408002) OT 1200 Fracture Table (sn: 0426113029) SURGIGRAPHIC Surgical Table (sn: 0426113029) STERIS 5085 Table (sn: 0433212099) STERIS 5085 Table (sn: 0432612116) STERIS 5085 Table (sn: 0433112033) STERIS 5085 Table (sn: 0433112034) STERIS 5085 Table (sn: 0433212098) V-PRO MAX (sn: 032811313) V-PRO MAX (sn: 033321202) located at the VA Central Iowa Healthcare Facility in Des Moines, Iowa. CONTRACTOR RESPONSBILITIES: Provide all equipment, materials, transportation, supervision and labor to accomplish work. Contractor shall comply with normal, safe working procedures as required and maintain VA Requirements for safety and infection control. Contractor will provide repair and preventative maintenance services to support the sterilizers, washers and operating tables. The sterilizers and washers are used by the SPS (Sterile Processing Service) technicians to sterilize instruments and other items used in surgeries and other procedures for patient care in support of VA mission and facility objectives. The vendor will comply will all applicable Federal Acquisition Regulations and VA Acquisition Regulations. SCOPE: Perform regularly scheduled inspections. Equipment is cleaned, checked and Adjusted as necessary. Susceptible components are routinely replaced to guard Against failure. Provide all part for routine repairs. All replacement parts, with the exception of catalytic bed for the Model 50A Abator, necessary during the term of the contract are provided at no charge to the government, in accordance with the agreement. Provide educational in-services and be thoroughly prepared to present certified in-service training program. Contractor will automaticly update any equipment that requires change due to engineering changes that have been established for sterilizers and abators at the hospital. In the event that the equipment requires extensive service which can best be at the contractor facility, a loaner unit will be provided at no charge to theVA. The loaner unit will allow the VA to maintain sterilization capability while repairs are being accomplished. GENERAL: The contract for the STERIS sterilizers, washers and operating tables will be for a base year (May 01, 2017 April 30, 2018) + 4 Option years.
