THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR QUOTE. The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI   53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Large business firms, to provide a Lumen Inspection System, brand name or equal to the INSPEKTOR ® Lumen Inspection System, for the William S. Middleton Memorial Veterans Hospital, 2500 Overlook Terrace, Madison WI 53705, as described below. General Requirements: BRAND NAME (INSPEKTOR ®) OR EQUAL: Salient Characteristics: Two (2) INSPEKTOR ® Flexible Scopes (p/n: 5001). 2 mm diameter. 350 mm length. Two (2) INSPEKTOR ® Mini Flexible Scopes (p/n: 5003). 1 mm diameter. 800 mm length. Four (4) USB Digital Cameras (p/n: 3002). Software included Four (4) 150 W Illumination Box (p/n: 4000). Four (4) Fiber Optic Light Cables (p/n: 4001). 1 mm. Four (4) Flex Arms (p/n: 6000). Four (4) Scope Clamps (p/n: 6001). Four (4) Table Clamps (p/n: 6002). Lumen Inspection System shall: Be capable to look through lumens as small as 1 mm in diameter. Be focus free. Have minimum of 9x magnification. Be waterproof. Be cleanable, at a minimum ability to high level disinfect. Include a camera option. Be mobile (able to move). Be direct view, with the ability to be connected to a computer. Be capable of inspecting da Vinci Xi EndoWrist instrumentation. Installation and on-site training required. All interested firms who can meet the requirements stated above should respond, in writing, INCLUDING ALL OF THE FOLLOWING: Company name. Address. Point of contact (name/title/telephone number/e-mail address). DUNS number. Tax ID number. FSS Contract number, if applicable. Indication of which business category (SDVOSB, VOSB, Small Business or Large Business) for which the organization qualifies. A capability statement that addresses the organizations qualifications and ability to provide the requirement depicted above, and proof (i.e. correspondence from manufacturer) of your firm being an authorized distributor for the manufacturer. Submit responses to the Contracting Officer, Thomas Chesak, at thomas.chesak@va.gov no later than 10:00 a.m. (Central Standard Time), 24 April 2017. VA69D-17-N-0946 Lumen Inspection System for Madison MVH shall be referenced on all correspondence regarding this announcement. TELEPHONIC INQUIRIES WILL NOT BE ACCEPTED. The NAICS Code 339113, Surgical Appliance and Supplies Manufacturing, is applicable to this acquisition; the size standard is 750 employees. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract. The Government will use responses to this notice to make an appropriate acquisition decision. This is the only notice, no other copies of this notice will be provided.
