1 each, SIGMA/TC3 Knee System, Model: Tc3 Femur: 96-00xx, MBT Revision Tibia 1294-35-xxx, Wedges: 1294-56-xxxx, Sleeves: 1294-54-xxx, Augments: 96-08xx, Sems: 86-7400, 86-7400, TC3 Inser: 96-23xx, Femoral Sleeves: 1294-53-xxx, Posterior Augments: 96-08xx. Quantity: One set of instrumentation and implants. These implants and instruments are needed for a patient specific surgery scheduled for 9 May 2017. Vendor rep must be available in the OR at the time of surgery. Shippinge charge is applicable. Please provide all documents which clearly describe the item(s) so that it/they can be evaluated by the end user. Failure to submit the requested substitute item information before the closing date listed above will eliminate the vendor from all further consideration. Items otherwise equal may not satisfy the requirement due to differences in material composition, size, environmental application, etc. If an item is furnished which does not meet the requirement the item will be returned at the vendor's expense for a full refund. Award will be made on the basis of the lowest price of proposals meeting or exceeding the acceptability standards for non-cost factors.
