Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-89 issued and effective July 14, 2016. This solicitation VA259-17-Q-0418 is issued as a request for quote (RFQ). The North American Industry Classification System Code (NAICS) for this procurement is 337127. This RFQ is being issued as a Veteran Owned Small Business set-aside under NAICS Code 337127, SBA Small Business Size Standard is 500 employees. Any Award Made as a Result of this Solicitation will be made on an All or Nothing Basis. All Items being Solicited (Products and/or Receivables as appropriate) must be of New Manufacture; No Remanufactured or "Grey Market" items allowed. FAR 52.212-4 applies to all Solicitations and Awards. Department of Veterans Affairs, VISN 19, VA Medical Center, 921 NE 13th Street, Oklahoma City, Oklahoma 73104, is seeking a quote for the items listed below, in accordance with the furniture requirements identified below. Solicitation and evaluation of offers are to be in accordance with (IAW) FAR 11.105 (a)(1). The requirement is for one hundred (100) each high back work chairs including the following specifications: High back Extended seat Synchronized free-float rocking Lockable in any position Integrated seat depth adjustment, side-actuated tension adjustment and independent back angle adjustment for computer work Ergonomic contouring of the seat shape and high-resiliency foam for the distribution of the user s weight evenly Lumbar support Chair adjustment allows you to move the backrest through an un precedented 6 range in 27 positions Fingertip control positions the lumbar support Cast aluminum base, standard black epoxy powder coated aluminum base Independent back angle adjustment, adjustable seat depth, upholstered backrest shell, height, width, and angle T adjustable arms. Chair Size: 23 ½-29 W x 21 D x 45 ½-50 ¾ H, Base diameter 26, Backrest upholstered 22 ¾ H x 18 W, Seat upholstered height from floor 16 ¼ -21 ½ H x 20 W x 18 ½ D. Warranty: Lifetime structural warranties, including mechanism, housing, cylinder housing, seat and backrest structures; ten (10) year warranty for all other parts of the chair including the fabric and cushions. Delivery Schedule: VA Health Care System can take immediate delivery of all items listed on order. Contractor shall schedule delivery within 1 week after the contract award, approved by the Point of Contact (405) 456-5461. VA Health Care System OKC shipping and receiving number is 405-456-5644.
