Description

Sources Sought Notice This is not a request for quotes or proposals. No formal solicitation document exists at this time. The Veterans Integrated Service Network (VISN) 4 provides Home Oxygen delivery and set-up to Veteran beneficiaries in Pennsylvania and Ohio Counties (see below for potential catchment areas). The Contractor(s) shall be responsible for providing Home Oxygen services/supplies to Veteran beneficiaries serviced by VISN 4 Prosthetics and Sensory Aid Service (P&SAS). The contractor shall provide all supplies, materials, equipment, transportation of equipment, set-up, labor, supervision, patient education, safety management, and infection control, as necessary, for patients on home respiratory care therapy. Potential sites for services include: James E. Van Zandt VAMC in Altoona, PA; VA Butler Healthcare in Butler, PA; Erie VA Medical Center in Erie, PA; and the VA Pittsburgh Healthcare System in Pittsburgh, PA. Altoona VAMC Catchment Area: Pennsylvania: Bedford, Blair, Cambria, Cameron, Centre, Clearfield, Clinton, Elk, Huntingdon, Indiana, Jefferson, Juniata, Mifflin, and Somerset Counties, West Moreland, Allegheny Butler VAMC Catchment Area: Pennsylvania: Allegheny, Armstrong, Beaver, Butler, Clarion, Forest, Lawrence, Mercer, Venango; Ohio Counties- Ashtabula; West Virginia Counties- Marshall; Erie VAMC Catchment Area: Pennsylvania: New York Counties- Chautauqua, Cattaraugus; Pennsylvania Counties- Warren, Venango, Crawford, Mercer, Forrest, Cameron, Potter, Jefferson, Kane; Ohio Counties- Ashtabula, Lake, Geauga, Trumbal Pittsburgh VAMC Catchment Area: Ohio counties- Belmont, Columbiana, Harrison, Jefferson, Mahoning, Portage, Trumbull, Muskingum; Pennsylvania Counties- Allegheny, Armstrong, Beaver, Butler, Clarion, Fayette, Greene, Indiana, Lawrence, Washington, Westmoreland, Venango; West Virginia Counties- Hancock, Marshall, Ohio, Tyler; Maryland Counties- Garrett This sources sought notice is issued in an attempt to determine the level of interest among qualified VETBIZ registered Service Disabled, Veteran Owned Small Businesses (SD/VOSB) and Small Businesses (SB) to provide the services described above. Only qualified VETBIZ registered SD/VOSB and SBs should respond to this notice. Responses from vendors other than SD/VOSB registered vendors or SBs will not be considered. Responses will assist VA planners in determining an appropriate acquisition and set-aside strategy which best fit VA requirements. Interested VETBIZ registered SD/VOSBs and SBs are requested to respond to this notice no later than April 25, 2017, 3:00 PM Eastern Time. With your response please provide a MAXIMUM two (2) page capability statement (additional attachments/pages will not be reviewed) summarizing experience and past performance in providing home oxygen services. Also include: Company Name, Point of Contact; Phone Number; Email Address; DUNS Number and Business Size. Interested vendors, please provide information regarding which sites, if not all, that your company would be capable of servicing.
