- The attached document are the attachments that are related to the Rate Quote Agreement and will require completion as stated in the Rate Quote Agreement RQ-245-D17-0001. The attached document is the rate quote agreement and is requesting for proposed rates per line item as listed, as well requesting for other information as stated. This is a combined synopsis/solicitation notice for the provision of Nonpersonal Healthcare Services - Physician and Other Professional Ambulatory Services for the Purchased/Referred Care (P/RC) Program. The Indian Health Service (IHS), Navajo Area Indian Health Service (NAIHS) intends to solicit sources to enter in Rate Quote Agreements under the Rate-Quote Methodology to provide the services stated above to IHS beneficiaries that are eligible for the P/RC program within the Indian Health Service (IHS) system in coordination with Federal Regulation. The nonpersonal healthcare services to be provided are for outpatient specialty health care services that are not available through the IHS delivery of health care; for example: cardiology, orthopedics, radiology, general surgery, oncology, podiatry, orthotics, prosthetics, dialysis, ophthalmology, eye surgery, urology, renal dialysis, etc. Services shall be performed at the Contractor's place of business to IHS P/RC Patients in outpatient and inpatient settings. General duties include independently providing the full range of health assessments, treatment, including prescribing medicatiosn if needed to the patients that are seen in order to accomplish the required tasks. The location for services shall be in the State of Arizona and New Mexico. Attachment D-4 of the Rate Quote Agreement package includes a full listing of the USPHS Service Units that are under the Navajo Area Indian Health Service (NAIHS) coverage. The Rate Quote Agreement package is attached and the NAIHS Division of Acquisition Policy will be accepting responses throughout the year of this notice. Responses shall be submitted as indicated on the cover letter of the Rate Quote Agreement package. All information for this notice shall be posted to the Federal Business Opportunities (FBO) website at www.fbo.gov. No hard copies of this notice shall be provided. All amendments to this notice will be available at this website, and offerors are advised that they are responsible for obtaining amended information prior to responding to this notice.
