Description

2 of 2 Dear Valued Vendors, Due to the Department of Veteran Affairs oversight of the Veterans Integrated Service Network (VISN) 2 Central Archive server, consisting of a primary site located in Bronx, NY and secondary site located in Albany, NY which will house a cardiology acquisition, archiving, storage and retrieval server effective April 12, 2017, Amendment A00001 is hereby issued by the National Acquisition Center to officially incorporate those two facilities in the Performance Work Statement, as follows: 2.4 Technical Requirements Delete: ¶ (2.4.1.2) p.16 Images shall then be uploaded to the VISN Central Archive server (VA site to be determined) which will house a cardiology acquisition, archiving, storage and retrieval server. Add: ¶ (2.4.1.2) p.16 Images shall then be uploaded to the VISN Central Archive server, consisting of a primary data site located in Bronx, NY and a secondary site located in Albany, NY, which will house a cardiology acquisition, archiving, storage and retrieval server. FINAL PROPOSAL REVISIONS The National Acquisition Center (NAC) has concluded discussions with all companies for Request for Quotation (RFQ) VA797N-15-Q-0001. Therefore, all vendors that are interested in this procurement must provide a refreshed Final Proposal Revision (FPR) on or before the closing date of April 26, 2017 NLT 3:30pm CST.. The Department of Veteran Affairs is providing opportunity only to those offerors that has previously submitted a RFQ on or before the closing date of 8-23-2016 the opportunity to refresh their Final Proposal Revision (FPR) by issuance of amendment A00001 thru FBO. In the (FPR) you may make corrections to your previous price offer and provide any clarifications, provide a technical refresh of your pricing only. All offers that fail to price all sites as requested will be disqualified. The Contracting Officer intends to make award without further discussion. (FPR) are due via electronic email to marlon.powell@va.gov. VISN 2 Central Archive Server Bronx, NY VAMC Primary site Albany, NY VAMC Secondary site All vendors interested in submittal of a Final Proposal Revision (FPR) must refresh the following excel spreadsheets and submit all changes with their (FPR). All vendors interested in submittal of a (FPR) must refresh and convert the word Offer submission document to PDF and submit all changes with their (FPR). Copy of VISN 2 Offer Submission word document spreadsheet with two additional sites Upon award the Recipient of award will be required to update and refresh all quoted facilities No options years for service and maintenance or optional pricing required FPR are due via electronic email to marlon.powell@va.gov using the attached SF1449 word spreadsheet convert the word spreadsheet to and PDF submit NLT April 26, 2017 at 3:00pm CST. No FPR will be accepted after 3:00pm CST on 4-26-2017.
