Description

This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA248-17-Q-0668 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-94, 2005-95/01-19-2017. Interested contractors may contact Christopher Brailer at 352-381-5730; emailed Requests for Information are preferred. Quotes should be sent to christopher.brailer@va.gov. To receive a government award, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. Online Representation and Certification Application (ORCA) must also be current to qualify to receive a contract award. NAICS and Set-aside: The NAICS is 339113 and the small business size standard is 750 employees. This procurement is set aside for Service Disabled Veteran Owned Small Businesses (SDVOSB). Any firm that is not considered as SDVOSB under this NAICS code should not submit a response to this notice. Only VA verified SDVOSB s in the VIP at the time of contract award will be considered. All non-VIP verified firms will be considered non-responsive and ineligible for award. The acquisition will be made pursuant to the authority in FAR 13 to use simplified procedures for commercial items. Contract Type: The government anticipates awarding a Firm Fixed Price award. Award will be made to lowest price quote considered fair and reasonable which conforms to the requirements within this solicitation. Description of Commodities: Patient Transport Chairs (SEE ATTACHED SALIANT FEATURES and PRIME TC SPEC SHEET). Goods tendered must be BRAND NAME OR EQUAL TO STRYER PRIME TC MODEL. Items shall be delivered FOB Destination to: Malcom Randall VAMC 1601 S.W. Archer Road Gainesville, FL 32608-1197 Solicitation document and incorporated provisions and clauses apply to this acquisition: Offerors shall provide the information required by FAR 52.212-1 (APR 2014), Instructions to Offerors-Commercial Items, which is incorporated by reference. If the end product (s) is other than domestic end product(s) as defined in the clause entitled Buy American Act Supplies, the offeror shall so state and shall list the country of origin. FAR 52.212-2 (OCT 2014) Evaluation Commercial Items- The following factors shall be used to evaluate offers: Products conformity to the attached salient characteristics Price Offerors, if not registered in SAM, must include completed copies of the provision at 52.212-3 (MAR 2015), Offeror Representations and Certifications - Commercial Items with their offer. The provision may be obtained via the internet at URL: http://farsite.hill.af.mil/reghtml/regs/far2afmcfars/fardfars/far/52_000.htm. These representations and certifications will be incorporated by reference in any resultant contract. FAR 52.212-4 (DEC 2014), Contract Terms and Conditions-Commercial Items is applicable. The following identified Federal Acquisition Regulations (FAR) and Veteran s Affairs Acquisition Regulation (VAAR) clauses are incorporated by reference: 852.203-70, 852.211-73, 852.219-10, 852.232-72, 852.237-70, 852.246-70, 852.246-71, 52.211-16, 52.216-1, 52.222-41, 852.270-1, and 852.273-74. FAR 52.212-5 (MAR 2015), Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items is applicable and the following identified clauses are incorporated by reference: 52.233-2, 52.203-13, 52.203-17, 52.204-9, 52.228-5, 52.219-27. Submitting Quote: Contractor shall submit their quote on company letterhead and must include, solicitation number, FOB destination to the Malcom Randall VAMC, proposed delivery schedule, discount/payment terms, warranty duration, and DUNS number. Quotes must be received April 17, 2017 by 1300 EST. All quotes must be e-mailed referencing VA248-17-Q-0668 to christopher.brailer@va.gov; Offers received after the exact time specified in the solicitation are considered late and may be considered if the action would not unduly delay the acquisition. All questions regarding this solicitation must be submitted to the Contract Specialist in writing (email is preferred). Any prospective bidder desiring an explanation or interpretation of the solicitation must request it via e-mail to the Contract Specialist: Christopher Brailer (christopher.brailer@va.gov) or by phone at 352-381-5730 no later than: close of business on April 12, 2017.
