Description

(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) Solicitation number VA261-17-Q-0386 is issued as a request for quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94/2005-95. (iv) This acquisition is a total Service Disabled Veteran Owned Small Business (SDVOSB) set aside. In order to be eligible for award the vendor must be registered in the VetBiz database. The NAICS code is 562998 and the size standard is $7.5 million. (v) The contract line item is listed in the attached price schedule of the solicitation package. When providing a quote, please provide a fixed rate for the line item. (vi) The contractor shall provide a service for the removal of radiology equipment for the Department of Veteran Affairs Palo Alto Health Care System in accordance with the attached Statement of Work. (vii) The contract will be for a period of performance based on 30 days ARO (After Receipt of Order). The period of performance is also shown in the Price Schedule and the Statement of Work. (viii) The provision 52.212-1, Instructions to Offerors Commercial, applies to this acquisition. (ix) The provision 52.212-2, Evaluation Commercial Items, does not apply to this acquisition. Evaluation will be based on price. (x) Offerors shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications Commercial Items, with its offer. (xi) The clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. (xii) The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders Commercial Items, applies to this acquisition. (xiii) All additional contract clauses and provisions are attached to this notice. (xiv) The Defense Priorities and Allocations System (DPAS) do not apply to this acquisition.
