REQUEST FOR INFORMATION/SOURCES SOUGHT NOTICE Radiofrequency (RF) Generator, manufactured by Halyard, PMG 115/230; Advanced, Version 4.11 Department of Veterans Affairs, VA Medical Center, (Jefferson Barracks Division) Pain Management Clinic, St. Louis, MO 1.0 Description 1.1 The St. Louis VA is seeking information to determine procurement SDVOSB/VOSB set-aside for providing a Radiofrequency (RF) Generator for the Pain Management Clinic at the VA Medical Center, Jefferson Barracks Division, St. Louis MO. The NAICS code is 339112. 1.2 THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. This RFI is issued solely for information and planning purposes - it does not constitute a Request for Quote (RFQ) or a promise to issue an RFQ in the future. This request for information does not commit the Government to contract for any supply or service whatsoever. Further, the VA is not at this time seeking proposals and will not accept unsolicited proposals. Interested Contractors are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this RFI; all costs associated with responding to this RFI will be solely at the interested party's expense. Not responding to this RFI does not preclude participation in any future RFP, if any is issued. If a solicitation is released, it will be synopsized on the Federal Business Opportunities (FedBizOpps) website. It is the responsibility of the potential offerors to monitor these sites for additional information pertaining to this requirement. 1.3 Interested parties are requested to respond to this Sources Sought Notice via e-mail only to to Karen Munroe, Contracting Officer; email: karen.munroe@va.gov no later than Monday, April 10, 2017, close of business. Proprietary information, if any, should be minimized and MUST BE CLEARLY MARKED. Please be advised that all submissions become Government property and will not be returned. 1.4 Interested Offerors shall provide, at a minimum, the following information: Name, mailing address, phone number, fax number, and e-mail of designated point of contact. 1.5. SDVOSB/VOSB, contractor must be registered with VetBiz Vendor Information Pages at www.va.gov/osdbu.
