Description
PLEASE READ: The purpose of this sources sought notice is to establish sources that currently have the capability and willingness to perform the solid waste and recycling management services required at the Phoenix VA Health Care System. The responses received will be used to determine any SDVOSB/VOSB or small business size standard set-aside that may or may not apply when the official Request for Quote (RFQ) is issued. The official RFQ will be issued on or about 5/31/2017. Below is the estimated Statement of Work with anticipated estimated quantities/frequency. ESTIMATED STATEMENT OF WORK CONTENTS Background Project Objectives Statement of Work/Requirements I. BACKGROUND The contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items and services necessary to perform (Non-Hazardous) Solid Waste Collection, Weighing, and Disposal Services at the Carl T. Hayden VA Medical Center, 650 E. Indian School Rd, Phoenix, AZ 85012. The contractor shall perform in accordance with all terms, conditions, provisions, schedule and attached scope of work (SOW) and resultant contract herein as well as all local, state, and federal regulations. Contract term will be for a base year (10/01/2017 09/30/2018), with four (4) one-year options. Award will be made subject to availability of funds. The C & A requirements do not apply, Security Accreditation Package is not required. Project does not involve the storage, generating, transmitting or exchange of VA sensitive information. No sensitive information will be handled by contractor performing work.. II. PROJECT OBJECTIVES: Remove solid waste and recycled materials from the facility III. SCOPE OF WORK/REQUIREMENTS: the contractor shall provide all necessary personnel, tools, equipment, and materials required to perform the tasks below. Unit 8 CU YD (8 ea.) pick up 2 x week 832 JB 40 CU YD Compactor (VA Owned w/ Dumpster contractor provided) 2 x week 104 JB 40 CU YD Open Top Roll-Off 1 x week 52 JB 40 CU YD Open Top Roll-Off Special Events 6 JB Recycling Collection Commingled plastic/metal/cardboard 8 CU YD (5 ea.) pick up 1 x week 52 JB 8 CU YD (1 ea.) Special Events 6 JB SOLIDED WASTE TASKS & FREQUENCY TABLE COLLECTION STATION LOCATION (SOLID WASTE) SIZE FREQUENCY 1 Pharmacy Loading Dock, North of Ambulatory Care Clinic 8 CU YD - 3 ea.
