RLP VA263-15-R-0285 Page 2 AMENDMENT NO. 1 REQUEST FOR LEASE PROPOSALS (RLP) VA263-15-R-0285 COMMUNITY-BASED OUTPATIENT CLINIC (CBOC) MINOT, ND C RLP VA263-15R-0285 is hereby amended as follows: (Revisions are noted in Bold and Italics.) In RLP Paragraph 1.14 BROKER COMMISSION (DVA), the last sentence of the second paragraph is hereby deleted in its entirety and replaced with the following: The total contract value that will be used to determine the three and one half percent (3.5%) commission will be established based on the final lease documents upon lease execution or as amended thereof. Such commission or lease acquisition fee shall be due and payable, as follows: RLP GSA Form R100, Cover Sheet, upper right side is deleted and replaced with: Offers are due by April 20, 2017. RLP GSA Form 100, Paragraph 1.06 is hereby amended by: Updating the number of pages for Section 8 Special Requirements, Exhibit A to 28 pages Updating the number of pages for Section 9 Space Plan, Exhibit B to 66 pages The addition of the PACT Space Module design guide JUNE 2015, Exhibit J, 95 pages. Offerors may locate this design guide at https://www.cfm.va.gov/til/dGuide/dgPACT.pdf. A hard copy of the document will be included in the lease at award. The addition of VHA Handbook 0730/4 dated March 29, 2013, Exhibit K, 22 pages. RLP Paragraph 2.13 KEY PERSONNEL EXPERIENCE AND PAST PERFORMANCE, Subparagraph C(2)(c) is hereby deleted in its entirety and replaced with the following: c. Submit the form no later than Close of Business (COB) on April 20, 2017. COB is 4:00 p.m. CDT. The first sentence in RLP Paragraph 2.13 KEY PERSONNEL EXPERIENCE AND PAST PERFORMANCE, Subparagraph (C)(2)(f) is hereby deleted in its entirety and replaced with the following: f. Sufficient information must be received by COB April 20, 2017, to be able to evaluate past performance. RLP GSA Form R100, Section 3, How to Offer, Paragraph 3.02 Receipt of Lease Proposals, subparagraph (B)(1) is hereby amended to reflect April 20, 2017, as the date for providing the offer and courtesy copy to the designated offices and addresses. A number of changes have been made to the GSA Form L100, Exhibit A, Exhibit B, and the Unit Price List. These documents are being reissued. The changes appear in red text within each document.
