- This Combined Synopsis/Solicitation has been amended on 29 March 2017. This amendment corrected the quotation due date within the Description section from 1 May 2017 at 10:00a HST to 27 April 2017 at 10:00a HST. All other terms and conditions remain unchanged. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Regional Health Contracting Office - Pacific, Bldg 339 Clark Road, Ft Shafter, HI 96858, intends to solicit, negotiate, and award Multiple Blanket Purchase Agreement for Lenses. This is a request for quotation (RFQ). This solicitation number is W81K02-17-T-0201. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The Standard Industrial Classification Code is 6515, size standard is 750 employees and the NAICS code of 339113. This procurement is Full&Open Competition. Provision at 52.212-1, Instructions to Offerors -- Commercial, and its addendum applies to this acquisition and a statement regarding any addenda to the provision. See Attachment1. Evaluation Shall be conducted in accordance with the procedures of section -- Commercial Item Evaluation Factors for Award. See Attachment1. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, and its addendum applies to this acquisition. See Attachment1. The deviation to clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. See Attachment1 for the FAR clauses that are applicable to this acquisition. The Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable. The Government intends to award Multiple Blanket Purcahse Agreements for a base period of 01 June 2017 through 31 May 2022 to the successful quoter(s). Contract Requirements - The successful offeror shall be required to meet the requirements of the attachment; Attachment1.
