Description

The Veterans Affairs NCO4, Contracting Department intends to procure via sole source with Medivators Inc, 14605 28th Ave N Minneapolis, MN 55447-4822 for a full service contract on Medivator units located at the Altoona VAMC. The applicable NAICS code for this procurement is 811219.   The size standard is $20.5 mil. The proposed contract action is for services which the Government intends to solicit and negotiate with only one source under the authority of FAR 6.302-1. Interested persons may identify their interest and capability to respond to the requirement or submit quotations. This notice of intent is not a request for quotations. Interested parties who believe they can meet this requirement must submit information about their capabilities no later than 12:00 PM EST, 04/04/2017.   All information shall be furnished at no cost or obligation to the Government.   No solicitation will be made available. Information must be sent to the Contracting Officer, April.Stanch@va.gov.     Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the Contracting Office will proceed with the sole source negotiation with Medivators, Inc. STATEMENT OF WORK (SOW) 1. Contract Title. Annual PM contract for 2 Medivator units, and a service contract covering both. 2. Background. Contract required for compliance with ISO 9000 Standards. 3. Scope. 1. Annual inspections MEDIVATORS REPROCESSING SYSTEM, ADVANTAGE Plus SS, 2.0 Model SN 80278490 BASE, AIR COMPRESSOR PRE-FILTER, W/LEAK TEST (COMPREHENSIVE). 2. Annual inspections MEDIVATORS REPROCESSING SYSTEM, ADVANTAGE Plus SS, 2.0 Model SN 80558203 BASE, AIR COMPRESSOR PRE-FILTER, W/LEAK TEST (COMPREHENSIVE). 4. Specific Tasks. For both Medivators: Corrective maintenance for service and support between the hours of 7:00 a.m. and 7:00 p.m., Monday through Friday with unlimited non-user error corrective maintenance. Includes parts, labor, and travel. Unlimited ADVANTAGE technical service representative support, including system diagnostics, telephone troubleshooting assistance and dispatch of on-site support as needed. Available between the hours of 7:00 a.m. and 7:00 p.m., Monday through Friday. Contractor shall provide planned maintenance in May as prescribed by the System User Guide. Includes parts, labor, and travel for annual preventive maintenance visits. Maintenance to be performed according to Manufacturer s Recommendations. Required: Service Order Reports Provide a detailed service report consisting of a description of the problem and an itemized list of parts replaced, as well as any labor action in order to keep the best track of your system s performance. Full Service Program Details Product System Improvements Mandatory and non-mandatory product system improvements are covered (does not include feature upgrades). Parts Discount N/A (All parts are included.) Vendor shall provide NIST (National Institute of Standards and Technology) traceable on-site calibration of process measuring equipment used in manufacturing and manufacturing-support environments. Vendor shall fill out and apply to a prominent surface of the apparatus an EQUIPMENT UNDER VENDOR SERVICE sticker. This green label can be obtained from the biomed workshop or by dialing x4142. The sticker must be completed with all following information: Vendor Name, Initials of Worker, Date Performed, and Re-Inspection Date (day, month, year). 5. Performance Monitoring Contractor service reports will be reviewed to ensure services completed as scheduled. Contracting Officer s Representative will discuss Contractor s performance with Medivators clinical users to ensure that issues are being satisfactorily addressed. 6. Security Requirements The Contractor will not disclose information incidentally ascertained from being in clinical areas. 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). There is no government furnished equipment or government furnished information in conjunction with this contract. 8. Other Pertinent Information or Special Considerations. a. Identification of Possible Follow-on Work. None anticipated. b. Identification of Potential Conflicts of Interest (COI). None perceived. c. Identification of Non-Disclosure Requirements. Incidental information gained from access to sensitive areas must not be disclosed. Vendor must agree to condition. d. Packaging, Packing and Shipping Instructions. No special requirements. e. Inspection and Acceptance Criteria. As previously noted under Performance Monitoring. 9. Risk Control Contractor is responsible for providing all applicable training and protective equipment to protect Contractor s employees from any hazards, including blood borne pathogens, which may be encountered in the performance of this contract. 10. Place of Performance. Generally, contractor is to perform PM and service on the 3rd floor of main building, SPD department, of James E. Van Zandt VAMC.
