Description

This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this sources sought must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses and Veteran Owned Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must contain sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) Company name (b) Address (c) Point of contact (d) Phone and email (e) DUNS number (f) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business and (g) Contractor must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. Purpose: The Phoenix VA Health Care System (Agency) needs to replace the pump on its single chamber washer/disinfector, the Steris Reliance ®Vision. Scope and Methodology: The Contractor will deliver the requirement in its entirety as listed below: Materials shall be new, not refurbished or previously used. Contractor shall ship and deliver all requirements as listed: (1) Steris Pump, pn: P117048900 Functional/Performance/Specification Statement and Evaluation Criteria NO. SPECIFICATION/CRITERIA RATING i Compatible with Steris Reliance ® Vision 3 ii Part Number: P117048900 3 iii Power: 2.6/10.4 KW 3 iv Voltage: 460-480 volt 3 v 1 year warranty 2 Legend: Rating 0: N/A 1: Low importance 2: Medium importance 3: High importance The Contractor must be a vendor approved and certified by the manufacturer, Steris, to work on the Steris Reliance Vision so to not void the current contract with the manufacturer. The Contractor shall cover travel and labor. The Contractor shall work Monday-Friday 7am-4pm LST. The Contractor shall check-in with the Agency before performing any work on-site. The Contractor shall remove and haul off the bad pump. The Contractor shall install the new pump. The Contractor shall conduct a post-installation quality assurance test on the washer. The work will not be accepted if the Steris Reliance ®Vision pump is not operating properly. The Contractor shall deliver a hard copy or soft-copy of a Field Service Report that delineates the service performed and confirms quality assurance (QA) test results. Notice to potential offerors: All offerors must register in the System for Award Management (SAM) at www.sam.gov before responding to any Government inquiry. Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who plan to submit an offer must register with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov.
