Description

This is a combined synopsis/solicitation for a commercial item prepared in accordance with all parts of and format in Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures in FAR Part 13. This announcement constitutes the only solicitation; a written solicitation will not be issued. This proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 13. Interested persons may identify their interest and capability to respond to the requirement or submit price quotations. This notice of intent is not a request for competitive price quotations. However, if there are two or more Service-Disabled Veteran-Owned Small Business (SDVOSB) and Veteran-Owned Small Business (VOSB) who are capable of providing a quote at a fair and reasonable price, this acquisition will be set aside for SDVOSB/VOSB. All vendors shall be registered in the System for Award Management (SAM) prior to award. Registration information is located at www.sam.gov. SDVOBs/ VOSBs not listed in https://www.vip.vetbiz.gov/ are considered non-responsive and price quotations will not be evaluated. Award will be made to the responsive/responsible offeror who provides the technically acceptable product at the lowest price. NAICS code 334510/1250 employees applies. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95, dated 19 Jan 2017. Only emailed requests received directly from the requester are acceptable. Email quotes to carol.lam@va.gov no later than the Monday, March 27, 2017. Receipt must be acknowledged on my desktop computer to be recognized as received. This is the only acceptable method for delivery of quotes. Please state if your company has any government contracts, i.e., GSA, SEWP, NAC FSS, BPA, or you plan on a submission under open market conditions. For any government contract, provide your contract number, terms and conditions; SIN the products fall under (if any), delivery schedule and contract expiration date.   VA Southern Nevada Healthcare System requirements are as follows: The VA Southern Nevada Healthcare facility has been treating Veterans with Prostate Cancer locally since 2006. A Pro-Seed Certified Radiation Oncologist in concert with a VA urologist and a VA Anesthesiologist has been treating Veterans with Prostate Cancer with Brachytherapy or radioactive seed implantation into the prostate to ablate cancer cells. The Pro-seed Certified Oncologist provides real time prostate implants utilizing a nomogram specific to the Bard Iodine 125 seeds. The Pro-Seed certified urologist determines the volume of the prostate by transrectal ultra sound in his/her office prior to the procedure with a special software program compatible to C.R. Bard. The volume measurements is then sent to the Pro-Seed certified radiation oncologist who enters the data into Versi-Seed software program specific to the C.R. Bard brachytherapy seeds to determine the number of seeds necessary to provide a prescription dose of no less than 160 Gy. The software program assures that the implant geometry and minimum peripheral doses are proper by specifying total seed activity for the given prostate volume. The number of Brachytherapy seeds is then sent to the VA Contracting Officer representative to order the seeds from C.R. Bard.
