Description

This announcement constitutes a Sources Sought Notice. This is not a Request for Quote (RFQ) or a Request for Proposal (RFP). This announcement is for information and planning purposes only and is not to be construed as a commitment by the government, implied or otherwise, to issue a solicitation or award a contract. Saint Cloud VA Medical Center (VHASTC), 4801 Veterans Drive, St. Cloud, MN has a requirement for a Firm Fixed Price Service Contract to be performed at the VA Medical Center campus as well as the three Community Based Outreach Clinics (CBOCs) located in communities outside of the VHASTC facility campus in St. Cloud, MN. The purpose of this notice is to determine interest and capability of potentially qualified businesses in particular Service-Disabled Veteran-Owned Small Businesses (SDVOSB) and Veteran-Owned Small-Businesses (VOSB) relative to the North American Industry Classification code (NAICS) 811219 Other Electronic and Precision Equipment Repair and Maintenance. To make an appropriate acquisition decision for this requirement, the Government will use responses to this sources sought synopsis. The type of solicitation issued and the manner of advertisement will also depend on the response to this sources sought synopsis. Description: Vendor to Furnish all Labor, Tools, and Equipment necessary to recycle organic waste from the VA Health Care System, 4801 Veterans Drive, St. Cloud, MN, for a period of 1 year to include 4 option periods as specified in the Statement of Work contained within this requirement. STATEMENT OF WORK (SOW) for Audiology Services Contract The following equipment residing at VHASTC requires calibration at the stated frequency. Qty 7 ea. - Madsen Astera audiometers, Frequency: Biannual Qty 7 ea. - GN Otometrics Otoflex 100 s Acoustic Immittance analyzer, Frequency: Annual Qty 7 ea. - Audioscan Verifits, or Aurical real ear if lifecycle replacement occurs within contract Frequency: Annual Qty 3 ea. - Capella OAE s, Frequency: Annual Qty 1 ea. - GSI Tympstar Acoustic Immittance analyzer, Frequency: Annual Qty 1 ea. - Lifeline International AT 235 Acoustic Immittance analyzer, Frequency: Annual Qty ea. - 1 GN Otometrics ICS Chartr 200 VNG and air caloric delivery system Frequency: Annual Qty ea. - 1 IHS ABR Frequency: Annual The following calibrations are required to be completed at each of the VHASTC CBOC s: Alexandria, MN CBOC: 515 22nd Ave E, Alexandria, MN 56308 Qty 1 ea. Madsen Astera audiometer, Telehealth Carts, Frequency: Annual Qty 1 ea. GN Otometrics Otoflex 100 s Acoustic Immittance analyzer, Telehealth Carts, Frequency: Annual Montevideo, MN CBOC: 1025 N 13th St, Montevideo, MN 56265 Qty 1 ea. GN Otometrics Otoflex 100 s Acoustic Immittance analyzer, Telehealth Carts, Frequency: Annual The following equipment are probable additions to required quantities to be serviced in FY 18 Qty 1 ea. - Madsen Astera audiometer Brainerd Community Outpatient Clinic expected onset service estimated FY 18, Frequency: Biannual Qty 3 ea. Aurical real ear verification systems, Brainerd Community Outpatient Clinic, onset service estimated FY 18, Frequency: Annual Required Standards: Audiometers must be calibrated to the American National Standards Institute Specifications for Audiometers: ANSI S3.6-1996 ANSI S3.1-1999 ANSI S3.39-1987 (R2002) Listed equipment other than audiometers are required to be calibrated in accordance with manufacturer specifications. Company must provide current NASED certification or equivalent program transcripts for all service technicians assigned to complete calibration services at any VA locations. Company must demonstrate prior satisfactory performance of the work requested in this requirement and provide references for that work. Written confirmation of service dates from the successful vendor must be communicated to the service chief and Biomedical Engineering contact two business weeks prior to the expected service date. Calibration services must be completed no later than one business day prior to the expiration of current calibration for all the above listed equipment with the exception of the Mobile Audiology Clinic van.. Mobile Audiology Clinic (MAC) VA21414 can only be serviced on a Monday. Any proposed alternative to service the MAC must be submitted, in writing to the Service Line Chief no later than 4 weeks (28 days) prior to the expiration of the current calibration. The proposed date will be no later than 2 weeks (14 days) prior to the expiration of the current calibration. Records of the calibration are to be provided no later than the end of the business day following the day the calibration work was conducted. -End of SOW: Submission requirements for responses to this sources sought notice: Interested vendors need to provide a letter of interest along with their capabilities statement.
