Description

This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA249-17-Q-0402 is issued as a (Request for Quotation (RFQ). This requirement is Total Small Business Set Aside) NAICS: The NAICS is 562991 and the small business size standard is $7.5 Million. Any firm that is not considered a small business under this NAICS code should not submit a response to this notice. To be receiving a government award, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. Online Representation and Certification Application (ORCA) must also be current to qualify to receive a contract award.. Offeror may register with SAM at www.sam.gov. Period of Performance (POP): The base POP is from the week of 3/13/2017 -6/13/2017 plus four monthly options if all options are exercised, ending 10/13/2017. Options may be exercised unilaterally by the Government. Description of Services: CLIN 0001 ADA = 2 STALL The contractor/vendor to provide winterized toilet trailers at the Tennessee Valley Healthcare System, VA Medical Center, 3400 Lebanon Road, Murfreesboro, TN 37129 in accordance with the scope of work. Work includes but not limited to: deliver and set-up two trailers, one ADA + 2 stall and one 4 stall trailer (these two trailers will be rented for 3 months with an option to extend on a monthly basis); provide 13 waste pumping services on each trailer; and pick up and haul away trailers when lease ends. 3 MO Unit Price Total CLIN 0002 4 STALLS 3MO Unit Price Total CLIN 0003 Delivery / Pick Up (for either size Trailer) 1 JB Total CLIN 0004 Pumping per Trip (for either size Trailer) Note: # of Pumping Trips TBD (depending on usage 1 JB Total CLIN 0005 Option Monthly 4 MO Unit Price ____________ Total_____________.GRAND TOTAL-------------------- Location of Performance: VA MEDICAL MURFREESBORO YORK CAMPUS 3400 Lebanon Road, ATTN: York/Engineering, Murfreesboro, TN 37129 1237 SOW: Provide winterized toilet trailers at Alvin C. York VA Medical Center to be set up in front of building 112 (CMOP). All work must be coordinated with the COR.
