The Government is only conducting market research and attaining information to identify contractors that are capable of providing all items/services listed below and the cost associated to acquire these specific items/services. This request for information (RFI) is strictly voluntary and is not to be interpreted as a quote in anticipation of a future contract award. 12. All questions and responses to this sources sought/ market research must be submitted in writing to Rosa G. Lara (rosa.lara.1@us.af.mil). Responses shall be submitted in MS Word or Adobe PDF format not later than Friday, 10 Mar 2017, 10:00am CST. 13. Respondent will not be notified of the results of the information. However, the Contracting Officer will utilize the information for technical and acquisition planning. All proprietary information not to be disseminated in Government documents must be clearly identified. Since this is a sources sought announcement only, no evaluation letters and/or results will be issued to the participants. This market survey is issued to determine the applicable planning and acquisition strategy based on responses indicating capability in responding to this requirement. -------------------------------------------------------- This is a follow-on requirement: Base + 4 (1 yr) Options with a Period of Performance of 15 October 2017 - 14 October 2022 APPENDIX A-1 - Physician 1. Qualifications: 1.1. The CP shall meet the following education requirements: Possess a Doctorate of Medicine (M.D.) or Doctorate of Osteopathy (D.O.) degree from an accredited university. Possess and maintain primary board certification by the American Board of Medical Specialties or a recognized specialty within Osteopathic Medicine. Current board certification in Undersea and Hyperbaric Medicine is required. Clinical experience in critical care/emergency care is highly desirable. Previous experience as a flight surgeon is also desirable. 1.2. Continuing Medical Education (CME). The CP shall provide appropriate CME documentation to the FRED within 1 month after all CME accredited events. 1.3. Shall have completed at least 20 CME hours in the past 12 months. 1.4. License and Certification Verification. (AFI 44-119, Medical Quality Operation (Chapter 4 Paragraph 4.6) 1.5. The CP shall have and maintain a current Basic Life Support (BLS) certification and Advanced Cardiac Life Support (ACLS) certification. 1.6. The CP shall also have and maintain a current Drug Enforcement Administration (DEA) registration number (certificate). License must correspond to DEA Registration number. 1.7. The CP shall provide all primary and annual/bi-annual licensure or certification document verifications. The CP shall maintain these certifications in a current status throughout the life of this contract. Verifications shall be submitted to the Government at least 10 working days prior IAW Appendix C. 1.8. The CP shall confirm that the clinical licenses or certifications have not been suspended or revoked by any healthcare facility, public or private, anywhere in the world. This does not preclude any CP whose licensure or certification, although originally suspended or revoked by a healthcare facility, were subsequently fully reinstated by the same. 1.9. The contract physician will maintain clinical privileges to treat Undersea and Hyperbaric Medicine patients as well as to perform wound care IAW the policies (by-laws) of WHASC Medical Staff by-laws, AFI 44-119, Army Reg 40-68, and Southwest General Hospital, as appropriate. Healthcare professionals who fail to maintain a license, registration or certification will be removed from independent practice and privileged providers will be placed in abeyance. If time exceeds 30 days of abeyance, the provider will be placed in summary suspension IAW AFI 44-119, paragraph 9.16. until the license, registration or certification is renewed. Issuing agency grace periods are not honored by the AFMS when renewing a license, certification, or registration. Should the CP be placed on summary suspension IAW AFI 44-119, the contractor shall replace the suspended CP to prevent any delay in service to the hyperbaric clinic. 1.10. The CP shall meet the following experience requirements: (a) Current board certification in Undersea & Hyperbaric Medicine, (b) formal fellowship training in Undersea and Hyperbaric Medicine, (c) working full time for at least 5 years of the last 10 years in Undersea and Hyperbaric medicine, and (d) experience in human subject research to include protocol writing, negotiating human subject research rules with respect to the Institutional Review Board (IRB), and publishing research. 1.11. Clinical experience as a staff physician in a graduate medical education setting and have at least 2 years' experience teaching Undersea and Hyperbaric medicine are highly desired. 1.12. Privileging Requirements. The credentials of the CP shall meet the privileging requirements as outlined in the most current version of AFI 44-119, Medical Quality Operations. 1.13. The CP shall furnish the MTF Credentialing Office with copies of valid, current, active, unrestricted medical licenses, registrations, education training certificates, and/or certifications IAW AFI 44-119, Medical Quality Operations. In addition, copies of board certifications are required, if applicable. The CP shall continuously maintain at least one active licensure or authorizing document from any U.S. jurisdiction throughout the life of this Contract. The CP shall be responsible for primary source verifications of all licenses and documents. 1.14. The CP shall screen applications for privileges through the National Practitioners Data Bank (NPDB), Healthcare Integrity and Protection Data Bank (HIPDB) by the CP. 1.15. The CP shall also screen through The Department of Health and Human Services (DHHS) at http://exclusions.oig.hhs.gov/ and TRICARE sanction listing at www.tricare.osd.mil/. The listings must be reviewed prior to entering into Contract or hiring CP for the position. 1.16. Professional staff appointments and clinical privileges are specific to the facility and the provider. An Initial Medical Staff appointment will be granted to all newly accessed CP for a period of 12 months. During this period, the CP performance will be under close review by their assigned clinical supervisor for clinical competence as well as for compliance with the facility's policies, procedures, bylaws, and code of professional conduct. Thereafter, the CP privileges will be reviewed/evaluated for an Active Medical Staff appointment, which is granted for a period of 2 years. 1.17. Healthcare professionals who fail to maintain a license, registration or certification will be removed from independent practice and privileged providers will be placed in abeyance. The CP shall implement clinic and 59th MDW policies, objectives and procedures in daily practice. 2. Duties: 2.1. Position Description: 2.1.1. Overseeing clinical hyperbaric treatment dives for routine and emergency patient care. The CP can recall the call team as required for emergency patient care. 2.1.2. The CP shall provide patients healthcare that meets the standard of care while expected to manage a typical caseload of 20-40 visits per week. 2.1.3. Conducting new and established patient evaluations, management and wound care. 2.1.4. The CP shall accurately & timely document medical record(s) within 3 business days of evaluation. 2.1.5. CP will perform Hyperbaric medicine consultant duties to DoD agencies for worldwide Hyperbaric treatment and operational support. 2.1.6. Participating in and supporting Hyperbaric and wound care research protocols. 2.1.7. Integrating into the call schedule for attending hyperbaric physician staff coverage. Cover after-hours on-call requirements consistent with current physician staff coverage policies. The CP will share call equitably with the other hyperbaric physicians. Generally, the CP physician is on-call 24/7 beginning at 1745 on Friday for one full week. Should there be a holiday, physician coverage is extended as determined by the Hyperbaric Medicine Flight Commander. Contract physicians should expect 15-17 weeks of call, but this could increase to 26 weeks per year to support manning needs. Accumulated "In-clinic" hours, to include after hours on-call requirements, will not exceed 1880 hours per fiscal year. Hours will be reviewed by the COR routinely. Recoverable hours will be arranged by section assigned senior military member. Staff required travel or official military duties will dictate the actual amount of weeks required to provide on-call services. 2.1.8. Participating in the education and training of Undersea and Hyperbaric Medicine fellows and other students including Residents in Aerospace Medicine (RAMs), other residents, medical students, nurses, and medical technicians. This consists of formal and informal presentations on wound care, Undersea and Hyperbaric medicine topics within the hyperbaric clinic or during local hyperbaric courses. The CP may assume the role of the Associate Program Director (APD) for the Undersea and Hyperbaric Medicine fellowship program, along with the attendant administrative and teaching duties. The APD position may require attendance at some graduate medical education meetings. 2.1.9. Assisting the Hyperbaric Medicine flight commander by attending various administrative meetings, providing input on policies impacting the day to day operations of the division, give both scheduled and impromptu tours of the Hyperbaric Chamber facility to visiting dignitaries, and providing educational lectures to various organizations in the local San Antonio area as well as at national meetings. The physician will also be encouraged to produce articles for publication in the hyperbaric medicine field and likewise offer assistance to the fellows in their publication endeavors as well. 2.1.10. Ensures a safe work environment, employee safe work habits and patient safety IAW regulatory agencies and facility specific safety guidelines (i.e. National Fire Protection Association, Code 99). APPENDIX A-2 - Licensed Vocational Nurse 1. Qualifications: 1.1. Current certification as Certified Hyperbaric Technologist (CHT) is highly desirable. If not a CHT, the CP shall obtain the CHT within 6 months of the date of hire at the contractor's expense. To obtain a CHT, the CP shall: complete a 40 hour introductory hyperbaric medicine course approved as a CHT qualifying course; 40 hours of a supervised clinical internship; 480 hours of hyperbaric medicine experience; Transcutaneous oxygen measurement (TCOMS) module with performance of three (3) TCOMS procedures; and pass the CHT exam. The MTF can provide the following criteria in the clinic to meet the CHT certification: 40 hours of a supervised clinical internship, 480 hours of hyperbaric medicine experience, performance of three (3) TCOMS procedures. The CP must have a current LVN certification IAW AFI 48-112. 1.1.1. Introductory Hyperbaric Medicine Course. CP shall be a graduate of a 40 hour introductory hyperbaric medicine course that is approved as a CHT qualifying course. If the CP is not a graduate of the aforementioned course, he or she will need to successfully accomplish this course and provide a certificate of training within 60 days of start of duty. Completion of this course is not available through the MTF and must be attained at contractor's expense. All fees associated with attendance of this course shall not be charged to the Government. The CP shall apply chamber safety to the everyday working environment and follow emergency procedure checklists in the event an emergency occurs. 1.2. The CP will possess current Basic Life Support (BLS) with Automated External Defibrillation (AED) certifications through the American Heart Association or American Red Cross. BLS/AED certification will be maintained during entire length of this contract. 1.3. Related nursing duties include background knowledge of Hyperbaric medicine, the dispensing of medications (to include narcotics), validating narcotic drug inventory, initiating, managing and discontinuing intravenous therapies, performing basic to advanced wound care procedures, basic patient care functions and clinical nursing Hyperbaric evaluations. 1.4. The LVN CP shall meet the following experience requirements: The CP will provide the appropriate licensure and have a minimum of 1 year of verifiable experience as a LVN. The CP shall be able to lift 40 lbs. 1.5. The CP shall have a working knowledge of computer use, Microsoft Office, and electronic health records. The CP must be able to provide in-service briefings. 1.6. The CP should have experience handling blood and body fluids and other potentially infectious material soiled patient linens. The CP will comply with each facility's infection control policies. 1.7. Possesses excellent written communication skills to perform accurate documentation, both written and electronic, of all activity IAW requirements. 1.8. The CP shall provide excellent customer service and communication skills. 1.9. The CP should possess skills and knowledge to perform physiological studies to measure patient health status to include, basic vital signs, pulse oximetry readings, transcutaneous oxygen monitoring studies, and ankle-brachial index study. 2. Duties: 2.1. Position Description: 2.1.1. The CP will obtain initial and routine pertinent medical information. The CP will be required to transfer medical history and physiological assessment data into various electronic medical health records. The CP shall accurately document data and patient history and findings into the electronic medical record within 3 business days of evaluation. The documentation must be clear and concise and easily interpreted by others that possess similar or advanced training and understanding in the medical community. The CP shall see patients of any age as long as they are eligible for care at the MTF. 2.1.2. The CP shall utilize customer service skills that are designed to optimize the interaction between the patient and the health care team. Customer service skills include personal interaction and communication skills, appropriate use of patient advocacy and resolution of customer complaints at the lowest level and elevation to patient advocate as necessary. 2.1.3. The CP shall perform patient care in a Hyperbaric and Wound Care Center. Daily responsibilities will surround the application of wound dressings, compression wraps, the appropriate knowledge and administration skills of topical, oral and parenteral medications, and the care required to support patients referred for care. CP shall also conduct basic and advanced wound care. Patient care may occur inside of a Class A Hyperbaric Chamber that is compressed to an approved treatment pressure. The CP shall perform hyperbaric chamber inside attendant duties as required for patient care, but not exceeding US Navy dive tables. The CP will be required to collect and appropriately label routine digital images of wounds and maintain a secure image library to account for patient wound progress. The CP will be accountable to maintain the laws governing patient confidentiality and the appropriate protection of that data and information. 2.1.5. The CP shall provide the general health care for patients with chronic medical needs to include, but not limited to diabetes, asthma, developmental disorders, ADHD, seizures, etc. 2.1.6. The CP shall complete medical record, vital signs, and patient assessment and questionnaires by the end of the duty day.
