- SCOPE OF WORK AND GENERAL SPECIFICATIONS FOR PLASTIC SURGICAL INSTRUMENTS Background: The Michael E. DeBakey VA Medical Center (MEDVAMC) is currently seeking to find services to provide targeted evidence-based dementia care coordination and assessment services to veterans and their family caregivers as described in the Telephone Assisted Dementia Outreach Program funded by the Office of Rural Health. The clinical tool being requested will facilitate service delivery and organize clinical data necessary for generating ongoing reports to the VA Office of Rural Health (ORH). Project Scope: The Contractor shall provide comprehensive in-person training by the program developer and two experienced staff members. This will include providing minor modifications to the Care Consultation Information System (CCIS) for TVAMC use, a CD for local installation and training on the Care Consultation Information System service delivery guide. The contractor shall provide all labor, supervision and other resources required to deliver the requirements stated herein, except as may otherwise be specified in this statement of work. The requirements include the following or equivalent: The contractor s work shall include Year One Initial Training and software installation for up to two Care Consultants, 1 Administrative Officer, and 1 Supervisor; Year One ongoing support and replacement staff training and production costs for manuals and marketing materials. Yearly renewal and updates after Year One will be required. Type of Contract: Firm-fixed price agreement with Yearly Renewal Option for continuing usage of the software licensing of Dementia Care Consultation for an additional two years, as well as software & manual updates, refresher training, consultation, and ongoing support with troubleshooting. General Requirements & Specific Mandatory Tasks and Associated Deliverables: See Table 1 below for Schedule of Services for the awarded contractor Initial training and software installation as described below (for 2 Care Consultants, 1 Administrative Officer and 1 Supervisor) should be completed within 30 days of Contract Award. Manuals and marketing materials are expected to be delivered at the same time. Table 1. Schedule of Services and Deliverables for Dementia Care Consultation License Task Description Year-One Licensing Initial training and software installation for up to two Care Consultants,, 1 Administrative Officer and 1 Supervisor Comprehensive in-person training by the program developer and two experienced staff members. Initial training costs are based on a two-day, 10 hour training session for no more than 10 participants, including no more than two Care Consultants. A longer initial training session and additional costs may be required for larger groups of participants and/or Care Consultants. The initial training can be held at the licensee's location or at the awarded contractor location to include installation and minor modifications of and training on the Dementia Care Consultation Information System, which is the computer software that guides service delivery.1 Year-one ongoing support and replacement staff training Ongoing/continuous technical support, up to two refresher training sessions by telephone, ongoing troubleshooting hotline, and training for replacement Care Consultants due to staff turnover during year one.2 Estimated at 30 hours during year 1. Production costs for manuals and marketing materials Up to five printed copies and an electronic version of manuals and accompanying training materials. Includes supplemental summary reports and fidelity monitoring tools and examples of marketing brochures and advertisements. Yearly Renewal and Updates After Year-One Permission for continued licensing the Care Consultation software for an additional one year renewal term, along with software updates; manual updates; half-day refresher training session; up to eight hours of service delivery consultation; and ongoing support/troubleshooting. Additional costs for training replacement Care Consultants after year one may be required. Warranty and Support. The warranty and support must cover all components of the requirement in accordance with standard manufacturer s warranty. Delivery Location: Warehouse Michael E.
