- Justification and Approval Pursuant to FAR Subpart 13.106-1(b)(1), the U.S. Army, MEDCOM, Regional Health Contracting Office-Pacific (JBLM-HCC), Lacey, WA, intends to issue a sole source award to Cochlear Americas Corporation the estimated amount of $7,000,000.00 for a Five Year Blanket Purchase Agreement to purchase Surgical BAHA( Bone Anchoring Hearing Aid) Implants and Accessories such as Baha 5 Sound Processor, Baha 4 Sound Processor, Baha BP110 Power Sound Processor, Baha Cordell II Sound Processor, Baha 4/5 Wireless Accessories, Baha Sound Processor Magnets and Software Pads, Baha Color Covers, Baha Softbands, Baha 5 Programming Equipment, Baha 4 Programming Equipment, Baha 5 Battery Doors, Baha ¾ Battery Doors, Baha BP110 Battery Doors, Baha Sound Processor Accessories, FAST Systems, Implants Abutments, Cover Screw and Implant Magnet, Surgical Components- Disposable, Attract Surgical Instruments-Reusable, Surgical Equipment, Vistafix Implants, Abutments and Surgical components, Prosthetic Lab Components, and Magnets tools, Nucleus Implant products such as but not specifically limited to; Profile, Hybrid, C122, Freedom Implants, Kanso and Nucleus Sound Processors, Surgical Equipment, Programming Equipment, Custom Sound Software, Covers, Batteries, Magnets, Controllers, consumable supplies, and accessories that will be used in patient specific surgeries located in the Department of Anesthesia and Operative Service at Madigan Army Medical Center (MAMC), JBLM, Tacoma, WA. Our surgeons are trained on these products and patients previously treated with these products must continue treatment with these same products. Prior to the surgery, the surgeon shall make the determination that these products or other branded products will best meet the individual patients' medical needs/requirements. All products must be FDA approved. This notice is not a request for quote/proposal and no solicitation will be issued. Any firm that believes it can provide the stated supplies/equipment can provide supporting documents demonstrating their products capabilities and specifications. Responses received will be used for future acquisitions. Information can be sent to the Purchasing Agent Ms. Kendre Green, kendre.m.green.civ@mail.mil. No telephone or facsimile requests will be accepted. This procurement will be prepared in accordance with FAR Part 13, using Simplified Acquisition Procedures.
