Description
SOURCES SOUGHT SYNOPSIS The Department of Veterans Affairs is issuing this sources sought synopsis as a means of conducting market research to identify parties having an interest in, and the resources to support, a requirement for injector maintenance at several locations in VISN 12. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 811219 Surgical instrument repair and maintenance services. The locations where services must be performed are: Edward Hines Jr. VA Hospital Jesse Brown VA Medical Center 5000 South 5th Avenue 820 S. Damen Ave. Hines, IL 60141 Chicago, IL 60612 Captain James A. Lovell FHCC Tomah VA Medical Center 3001 Green Bay Road 500 E. Veterans St. North Chicago, IL 60064 Tomah, WI 54660-3105 William S. Middleton Memorial Veterans Hospital 2500 Overlook Terrace Madison, WI 53705-2254 *It is the intention of the Department of Veterans Affairs to issue one RFQ which would include injectors at all five of the above locations. However, if an interested party can only perform at some of the locations above, the Government could issue more than one RFQ for the requirement. Such a decision is under the discretion of the Contracting Officer. THERE IS NO SOLICITATION AT THIS TIME. This request for capability information does not constitute a request for proposals; submission of any information in response to this market survey is purely voluntary; the government assumes no financial responsibility for any costs incurred. Tentative requirements: Provide maintenance and service for several types of Bayer and/or Boston Scientific Injectors to include, Stellant, Solaris, Angiojet Ultra, and Provis. The service agreement will include one (1) annual Preventive Maintenance task, and emergency work as well. If your organization has the potential capacity to perform these contract services, please provide the following information: 1) Organization name, address, email address, web site address, telephone number, and size and type of ownership for the organization. 2) Tailored capability statements addressing the particulars of this effort. The government will evaluate market information to ascertain potential market capacity to provide services consistent in scope and scale with those described in this notice and otherwise anticipated. BASED ON THE RESPONSES TO THIS SOURCES SOUGHT NOTICE/MARKET RESEARCH, THIS REQUIREMENT MAY BE SET-ASIDE FOR SMALL BUSINESSES OR PROCURED THROUGH FULL AND OPEN COMPETITION. Telephone inquiries will not be accepted or acknowledged, and no feedback or evaluations will be provided to companies regarding their submissions. Submission Instructions: Interested parties who consider themselves qualified to perform the above-listed services are invited to submit a response to this Sources Sought Notice by 4:00 PM EST on February 27th, 2017.
