VA256-17-N-0137 This is a Sources Sought Notice with the intent of gathering information and data as part of Market Research. This is not a request for quotes/proposals; request for quotes/proposals will be issued at a later date. No Award will be issued from this notice. This notice closes 03 March 2017 at 4:00PM CST. Information is to be emailed to the Contacting Officer at martin.priest@va.gov. The Southeast Louisiana Veterans Health Care System (SLVHCS) in New Orleans, Louisiana is seeking Adult Day Health Care Services for veteran beneficiaries in the New Orleans, Louisiana area. The ADHC facility shall ensure that care meets the needs and promotes the maximum well-being of VA patients. This facility will provide Nursing Services, Medical Services, Social Services, Rehabilitation Services, Nutrition Services, Recreation Services, Transportation Services, and Assistance in Activities of Daily Living, Case Management, and Patient Record Management. This is planned to be a base contract with four (4) one year option periods. Contract period will not exceed five (5) years.
