Description

Statement of Work Overton Brooks VA Medical Center Clinical Reference Database Background: Overton Brooks VA Medical Center located in Shreveport, LA requires print subscription and online access to two periodicals, Pharmacist s Letter and Pharmacy Technician s Letter. The purpose will be to provide medical staff access to a variety of medical research references material. Period of Performance: One year Base Period: February 1, 2017 January 31, 2018 Contract Type: Firm Fixed Price Scope of Work: The contractor shall provide secure web-based access for concurrent users (35 for Pharmacist s Letter with CE and 14 for Pharmacy Technician s Letter) with CE. The Contractor shall provide access to titles all via a single web portal. Post Award Testing/Acceptance: Within five business days of award the Contractor shall establish all required login accounts and fully demonstrate to the Government that sessions are able to be accessed concurrently. The Contractor shall also demonstrate that all required database resources are fully accessiable. The COR shall forward notice to the Contractor upon achieving successful access to the required databases. The notice shall be sent via email to the Contractor and the Contracting Officer and shall shall serve as acceptance by the Government in accordance with paragraph 5. Availability/Site Access: The Contractor shall provide web-browser based access for secure and concurrent sessions within five (5) business days of contract award. Access shall be minimally accomplished using Internet Explorer version 9. The database shall be available 24 hours per day, 7 days per week including weekends and holidays. Maintenance: Notwithstanding the requirements of paragraph 6, the Contractor shall be authorized up to five hours downtime per month for routine maintenance. The Contractor shall notify the COR via email at least five days prior to scheduled maintenance activities. Key Personnel: Contractor shall provide names of employees responsible for contract performance. The Contractor shall update this information as needed throughout the performance period. The Contractor shall designate and identify a primary and alternate program or project manager authorized to make decisions and oversee contract implementation and day-to-day operations. The Contractor shall provide names, telephone number, and email addresses for each individual named. Technical Support: The Contractor shall provide a technical support number and email address for end user trouble calls, both shall be clearly observable on the Contractors web portal. The support center shall be staffed by a live person between the hours of 8:00 a.m. and 4:30 p.m. CT Monday Friday. The Contractor shall ensure that logins are accomplished using IP authentication. Quality Assurance Surveillance Plan (QASP) The Contractor will evaluate performance in accordance with the QASP. Reports: The Contractor shall provide to the COR a monthly utilization report detailing the amount of time each resource was utilized during the previous month. The reports shall also clearly identify web-portal down time. Down times shall be segregated between scheduled and unscheduled maintenance. The report shall be submitted in PDF format unless otherwise expressly directed by the COR, or the Contracting Officer. The report shall be deliverd via email to the COR no later than the 10th business day of each month. In accordance with FAR 37.114(c) all documents or reports produced by the Contractor shall be suitably marked to clearly show they are Contractor products, or that the Contractor participated in creating the document or report. Reports shall be at no additional cost to the Government. Contract Administration: The Contracting Officer is the only person authorized to approve changes or modify any requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes that affect price, scope, quantity or quality of performance of this task order. In the event the Contractor effects any such changes at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in cost incurred as a result of such actions.
