Description

This is a combined synopsis/solicitation for commercial items/service prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation in its entirety; quotes are being requested and a written solicitation will not be issued. This is a request for quotes (RFQ) under solicitation number VA262-17-Q-0438. The solicitation document and incorporated provisions and clauses are in effect through Federal Acquisition Circular 2005-74 & 75 effective May 30, 2014 & June 24, 2014, respectively. The solicitation is in accordance with FAR Part 12, 13, and 19, Acquisition of Commercial Items, Simplified Acquisition Procedures, Small Business Programs, and Veteran Affairs Acquisition Regulations as supplemented with additional information in this notice. This is a 100% Service Disabled Veteran Owned Small Business (SDVOSB) set-aside. The North American Industrial Classification System (NAICS) code for this requirement is 334510; size standard 500 employees. The Small Business Administration has granted a Non-Manufacturing Rule Class Waiver for the selected NAICS code. The Department of Veterans Affairs Veterans Integrated Services Network (VISN) 22, headquartered in Greater Los Angeles, California has a requirement for: The VAGLAHS requires Ninety (90) Mobile Cardiac Outpatient Telemetry (MCOT), Holter Monitoring and CardioKey (extended 14 day holter monitor) devices for patient care in the Cardiology Section of the Department of Medicine. This is a request for a reimbursement rate cost per service. We are not looking to purchase the equipment, after the evaluation period the patient shall return the device to the vendor.
