Description
The VA Great Lakes Consolidated Mail Outpatient Pharmacy (CMOP) processes high volume, outpatient mail prescription workloads using integrated, automated dispensing equipment located at Building 37, Northwest Dock, West Hines, Illinois 60141. The Great Lakes CMOP has a recurring requirement for an Authorized Service from a NAID Certified Contractor responsible for shredding and properly destroying sensitive, classified/HIPPA data paper, ribbon, documents, and Mylar type bags. Contractor shall pick up material once a month. Contractor shall have a security process that involves providing eight (8) each 95 gallon and nine (9) each secure 64-gallon movable storage containers that can be used to store sensitive, Personal Health Information (PHI), and Personally Identifiable Information (PII) material awaiting pickup for destruction. This material averages about 500-600 pounds monthly, upwards to1,200, thus requiring additional pickups when needed. Destruction of materials shall take place off-site and documented to meet VA Standards. The purpose of this contract is to provide shredding (classified/HI-WA type paper and Mylar type mailing bags). Contractor shall be required to wet pulp, macerate, chop, and shred or otherwise definitively destroy the information contained in the records to a level that it is not readable or reconstruct able. The destruction of the information shall be witnessed by the Contractor and a documented certification of destruction shall be provided to VA. The resulting contract for this solicitation will be a firm fixed price, indefinite delivery contract (IDC) for the duration of 1 March 2017 through 28 February 2018 with four options to renew. In response to this solicitation, please provide the following: Name of the firm, point of contact, phone number, email address, DUNS number, and a statement regarding small business status certifications for SDVOSB, and the corresponding NAICS code. Capability statement to identify ability to meet government requirements. Examples of signed receipt, certificate of destruction, and containers to be provided. Suggested monthly off-site schedule dates and times, as well as how much notice is needed for additional pickups. Pricing options flat rate fee versus by weight charge. This is a 100% set aside for SDVOSB.
