Description
This is a SOURCES SOUGHT NOTICE in support of the VA PUGET SOUND HEALTH CARE SYSTEM in Seattle, WA for market research purposes only to determine the availability of potential businesses with capabilities to provide the services described below. Potential offerors are invited to provide feedback via e-mail to Robert E. Lentz at robert.lentz2@va.gov. Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. All responses due by February 08, 2017, at 1500 PDT. Potential contractors shall provide, at a minimum, the following information: 1) Company name, address, and point of contact, phone number, e-mail address, and DUNS. 2) Is your firm eligible for participation in one of the following small business programs? The Capabilities Statement for this sources sought is not expected to be a Request for Quotations, Request for Proposals or Invitation for Bids, nor does it restrict the Government to an ultimate acquisition approach, but rather the Government is requesting a short statement regarding the company s ability to provide the services outlined in the draft SOW below. Any commercial brochures or currently existing marketing material may also be submitted with the capabilities statement. Submission of capabilities statement will assist our office in tailoring the requirement to be consistent with industry standards. The capabilities will be evaluated solely for the purpose of determining to Set-Aside for the Small Business (SB) Community or to conduct as an Unrestricted Procurement. Other than small businesses may respond to this notice in the event the market does not indicate SB interest. This synopsis is for information and planning purposes only and is not to be construed as a commitment by the Government. The Government will not pay for information solicited. Respondents will not be notified of the results of the evaluation. Note: Do not include Proprietary, classified, confidential, or sensitive information in responses. Not responding to this Sources Sought does not preclude participation in any future or potential solicitation, or Request for Quotation. It is the intent of the VA PUGET SOUND HEALTH CARE SYSTEM to use the information gathered for market research purposes only. If a formal solicitation is released, it will be posted via the Federal Business Opportunity (http://www.fbo.gov). STATEMENT OF WORK 1. DESCRIPTION OF THE REQUIREMENT: ELEKTA IMPAC MOSAIQ OIS PRODUCTS COVERED BY STANDARD SOFTWARE AND SERVICE MAINTENANCE & SUPPORT CONTRACT VA PUGET SOUND HEALTH CARE SYSTEM. EQUIPMENT IDENTIFICATION SERVICE CONTRACT for March 1, 2017 through February 28, 2022, with potential for option years. SEE SPECIAL REMARKS FOR LIST OF TERMS REQUESTED IN THIS CONTRACT. 2.A. Contractor shall furnish all necessary travel, labor, materials, tools, documentation, and parts required for the inspection, support, and repair of the equipment, software, and accessories listed in the Schedule of Equipment, located at the VA Puget Sound Health Care System, American Lake division. This contract shall cover for a Base-Option (year) and 4 Option Periods (year) from MARCH 1, 2017 through FEBRARY 28, 2022 during the designated hours of coverage, in accordance with all terms, conditions, provisions, schedules, and specifications of this solicitation. 3. DEFINITIONS/ACRONYMS 3.A. VA Department of Veterans Affairs Medical Center, also known as the VA Puget Sound Health Care System. VAPSHCS VA Puget Sound Health Care System. Two divisions of the VAPSHCS: Seattle and American Lake. 3.A.1. VAPSHCS Seattle Division, 1660 S. Columbian Way, Seattle, WA 98108. 206-762-1010. 3.A.2. VAPSHCS American Lake Division, Tacoma, WA 98493. 253-582-8440. 3.A.3. Biomedical Engineer(ing )- Biomedical Engineer or designated staff (138-BIO). Seattle Division 206-764-2272, room 243 building 1 and American Lake Division 253-582-8440 x72188, room D3 building 18. 3.A.3. CO Contracting Officer of the Department of Veterans Affairs, Visn 20. 3.A.4. COTR Contracting Officer's Technical Representative, Biomedical Engineer or designated staff, Seattle Division 206-764-2272, room 243 building 1 and American Lake Division 253-582-8440 x72188, room D3 building 18. 3.A.5. PM - Preventive Maintenance. Services which are periodic in nature and are required to maintain the equipment in such condition that it may be operated in accordance with its intended design and functional capacity with minimal incidence or inoperative conditions. 3.A.6. FSE - Field Service Engineer. A fully qualified individual who is authorized by the contractor to perform maintenance (corrective and preventive) services on the VAPSHCS premises as defined in the terms of this contract. 3.A.7. ESR - Vendor Engineering Service Report (Service Tickets). Documentation of the services rendered for each incidence of work performance under the terms and conditions of the contract. 3.A.8.. Acceptance Signature - Signature by VAPSHCS employee indicating acceptance of FSE work completion or pending status as stated on ESR. 3.A.9. Authorization Signature - COTR's signature accepting work as stated on the ESR. 3.A.10. Glassware - radiographic emission and image intensifier tubes, unless otherwise stated. 4. INFORMATION SECURITY: 4.A. Information Security: If the Contractor has access to patient information or hardware containing patient information, the contractor must follow VA NW Health Network (VISN 20) Standard Operating Procedure 1.3.h Contractor Background Security Investigations as well as VA national information security standards referred to in the SOP including: VA Directive 0710, Personnel Suitability and Security Program; VA Directive 6500, Information Security program; VA Handbook 0710, Personnel Suitability and Security Program; Information Letter 90-01-6, Contractor Personnel Security Requirements; Notice 06-6, Procedures for Initiating Background Investigations for Contractors. If using VPN connection the contract must follow VA information technology standards for gaining VPN access. Other VA approved alternative language may be acceptable on approval. 5. SERVICE DESCRIPTIONS: The contractor will provide the products, upgrades, services, and enhancements as described below. 5.A. SOFTWARE UPGRADES 5.A.1. Purpose The Contractor will provide product Software Upgrades for the system above to keep the software system current with the latest features and enhancements. 5.A.2. Scope 5.A.2.a. The Mosaiq Supplier, Elekta, releases at least one major upgrade every year with significant feature enhancements. Upgrades are managed by Contractor upgrade specialists, working in collaboration with information technology personnel at customer sites. Upgrades will be performed remotely and scheduled to ensure little or no downtime for the facility. 5.A.2.b. System Administrator: The facility will have a dedicated and trained System Administrator at the facility. The System Administrator should have a basic knowledge of the applicable software product. The System Administrator must complete all Readiness Checklists to initiate the Upgrade per the Upgrade procedure and ensure that the Hardware Requirements required for the release. 5.B. FACTS ELECTRONIC SERVICE BULLETIN 5.B.1. Purpose The purpose is to provide the VAPSHCS with notification on current and pending software upgrades, upcoming events and training opportunities and important service announcements. 5.B.2. Scope Supplier s eFACTS Service Bulletin should be distributed by email every two months to the VAPSHCS. There will be no limit to the number of individuals at a customer site who may register for eFACTS; however, a validemail account is required. VAPSHCS will update contact information using the My IMPAC section of 5.B.3. SupportPlus: https://supportplus.impac.com. 5.C. SERVICE ALERTS 5.C.1. Purpose: The Contractor will provide Service Alerts will provide information regarding critical support issues. 5.C.2. Scope The Contractor will provide Service Alerts are distributed by email to the VAPSHCS. There is no limit to the number of individuals at a VAPSHCS site who may register for Service Alerts; via a valid email account. VAPSHCS will provide contact information via SupportPlus: https://supportplus.impac.com. 5.D. TELEPHONE SUPPORT 5.D.1. Purpose: The purpose of Telephone Support is to provide a dedicated support representative to assist VAPSHCS customers with questions and ensure maximum system use. 5.D.2. Scope The Contractor will provide phone support to all VAPSHCS customers on a Standard or Premium support contract during regular business hours, Monday through Friday from 8:00am to 5:00pm, local time. All phone correspondence will tracked in the customer tracking system and monitored to ensure timely response within two business hours or less. 5.E. EMAIL SUPPORT 5.E.1. Purpose The purpose of Email Support is to provide a dedicated support representative to assist customers with questions and ensure maximum system use. 5.E.2 Scope Contractors will provide phone support to all VAPSHCS customers through support@impac.com. All email correspondence will tracked in the customer tracking system and monitored to ensure timely response. 5.F. USER MEETINGS 5.F.1. Purpose The purpose of User Meetings is to provide customers worldwide an opportunity to learn more about using the features of the IMPAC System to maximize personal efficiency. User Meetings also provide an opportunity to network with other customers and employees who develop, install and service the IMPAC system. 5.F.2. Scope The Contractor will provide one annual System User Meeting per year. There is no limit to the number of individuals at a VAPSHCS site who may attend a User Meeting; however, travel expenses will the responsibility of the VAPSHCS. 5.G. REGIONAL COMMUNITY PROGRAM 5.G.1. Purpose The purpose of the Regional Community Program is to develop regional customer networks. Regional meetings are organized by regional Contractor customer representatives (Host Sites). The Contractor will underwrites the cost of these events through Host Site Educational Grants. 5.G.2. Scope There will be no limit to the number of individuals at a customer site who may attend a Regional Community meeting; however, travel expenses are the responsibility of the VAPSHCS. 5.I. ONLINE SUPPORT - SUPPORTPLUS 5.I.1. Purpose The purpose of SupportPlus (https://supportplus.impac.com) is to provide an Internet-based support system. 5.I.2. Scope The Contractor will provide SupportPlus, and all the features describe herein, is available 24 hours a day, 7 days a week. There will be no limit to the number of individuals at a VAPSHCS who may register for and use SupportPlus; however, the Contractor will provide a username and password as required for every session. 5.I.3. The following Support Plus Features will be provided: 5.I.3.a Support Plus: My IMPAC The purpose of the My IMPAC section of SupportPlus is to provide direct access to the VAPSHCS account. Using My IMPAC, the VAPSHCS can access up-to-date account information regarding installed products, maintenance and support status, staff records, network profiles and service calls. 5.I.3.b. SupportPlus: Software Updates The purpose of the Software Updates section of SupportPlus is to issue minor updates ore patches that provide additional features between major software releases. 5.I.3.c. SupportPlus: Documentation Library The purpose of the Documentation Library section of SupportPlus is to enable online access to the full product documentation suite, including release notes, checklists, product notes, reference manuals, technical requirements and training materials. Translated documentation is also available. 5.I.3.c. SupportPlus: Knowledgebase The purpose of the Knowledgebase section of SupportPlus is to provide database of application and technical support questions and answers. Supplier adds new questions and answers based on the information gathered working with clients in the installation, training and support process. 5.I.3.d. SupportPlus: Online Demonstration System The purpose of the Online Demonstration System section of SupportPlus is to allow customers to view new versions of software in pilot production. Customers can test new features in advance to better prepare for the next upgrade. 5.I.3.e. SupportPlus: Training Videos The purpose of the Training Videos section of SupportPlus is to further educate staff on using the IMPAC system. Videos range from 10-15 minutes and provide instruction on specific feature sets. 5.I.3.f. SupportPlus: Support FAQs The purpose of the Support FAQs section of SupportPlus is to help customer review common support questions the quickest way possible. Supplier regularly compiles and publishes the top 10 support questions for every software product line. 5.I.3.g. SupportPlus:Product Suggestion Form The purpose of the Product Suggestion Form of SupportPlus is to allow customers to submit product feedback directly to Supplier product managers. 6. Records Management Language for Contracts Required The following standard items relate to records generated in executing the contract and should be included in a typical Electronic Information Systems (EIS) procurement contract: 6.A. Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. 6.B. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. 6.C. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. 6.D. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. 6.E. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. 6.F. The Government Agency owns the rights to all data/records produced as part of this contract. 6.G. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. 6.H. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. 6.I. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. 6.J. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
