Attachment 2- Pricing Schedule/Medical Supply List Statement of Work Sources Sought The Department of Health and Human Services (HHS), Assistant Secretary for Preparedness & Response, Office of Acquisitions Management Contract and Grants (AMCG), is seeking information concerning the availability of offerors who can supply medical supplies. ASPR provides emergency medical care to the victims of public health emergencies and catastrophic events that overwhelm the capacity of state and local emergency medical systems to respond to these disasters. ASPR currently has teams located across the nation that are prepared to deploy within hours to respond to an all hazards environment. ASPR has positioned medical supplies caches/kits at various locations around the Continental United States, Alaska, Puerto Rico, and Hawaii. Medical caches for Disaster Medical Assistance Teams, International Medical Surgical Response Teams, National Veterinarian Response Teams, Disaster Mortuary Teams, Rapid Deployment Teams, and other Response Teams are located within the warehouses of ASPR. ASPR requires a wide variety of medical supplies that are purchased then subsequently packaged into pre-configured caches/kits that are maintained by ASPR staff in preparation for an emergency. During an emergency, ASPR requires additional medical and surgical supplies for purposes of re-supplying caches/kits. The exact medical supply items, quantities and delivery requirements are not known in advance and may vary considerably for re-supply. The delivery locations during response operations will be determined at time of need. The contractor shall have the capability to either use internal delivery systems or drop-shipments based on accessibility to locations. This Sources Sought is issued for informational, market research, and planning purposes only. This is not a Request for Proposal (RFP) or promise to issue a solicitation in the future. In accordance with FAR Part 15.201(e), responses to this notice are not considered offers, shall not be used as a proposal, and cannot be accepted by the Government to form a binding contract. We are not seeking proposals at this time. Not responding to this RFI does not preclude participation in any future solicitations, if one is issued. HHS will not be responsible for any costs incurred by interested parties in responding to this Sources Sought. Contact with Government personnel other than those specified in this Sources Sought by potential offerors or their employees regarding this project is prohibited. This FedBizOpps notice is an announcement seeking market information on vendors capable of providing the supplies as described herein. The information requested by this Sources Sought will be used within HHS to facilitate decision making and will not be disclosed outside the agency. The North American Industry Classification System (NAICS) Code is 423450, Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers and the small business size standard is 200 employees. Interested parties shall have access to at least 80% of the items listed on the master supply list at any given time to be able to ship to different locations in the Continental United States, Alaska, Puerto Rico, and Hawaii. If your firm is capable of providing the requirements described above and contained the two attachments, please provide responses to Cheryl Perdue at cheryl.perdue@hhs.gov and Carol Allen at Carol.allen@hhs.gov by February 13, 2017 @ 8:00 PM EST. At a minimum the following information is requested: Company Cage Code, DUNS, name, business size, address, point of contact, telephone number, e-mail address. Also interested parties need to state if they will be able to perform the required work, supply 80% of the master list at any given time, and be able to supply items in time of emergencies in accordance with the DRAFT Statement of Work. Additional Information This requirement is expected to be a Firm Fixed Price Commercial IDIQ for a Period of Performance of a one (1) year base and four (4) one year options. Maximum/Minimum IDIQ Limitation: The basic IDIQ maximum dollar amount is expected to be $19.9M and the minimum dollar amount $2,000.00 for the base and four (4) option periods.
