- THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR QUOTE. The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Large business firms, to provide two (2) ventilators with battery packs and roll stands that are Brand Name or Equal to the Philips Respironics Trilogy 202 (Item Number: 1040007) for the Veterans Affairs Illiana Health Care System (VA IHCS), 1900 E. Main Street, Building 19, Danville, IL 61832-5198, as described below. BRAND NAME (PHILIPS) OR EQUAL: Salient Characteristics: Two (2) Philips Respironics Trilogy 202 Ventilators (Item Number: 1040007). Ventilator is consider an invasive life-critical system and shall: Be a mechanical and reliable system. Move breathable air into and out of patients lungs. Provide breathing for a patient who is physically unable to breath or breathing insufficiently. Be portable. Have a back-up power source (backup battery). Have a reliable power supply. Have safety valves. Be equipped with monitoring and alarm systems for patient-related parameters and ventilator functions: Pressure, volume, and flow. Air leakage, power failure, mechanical failure. Have the ability to be non-invasive - Continuous Positive Airway Pressure and Bi-level Positive Airway Pressure (CPAP/Bi PAP). Have monitored Parameters. Include controls for: Pressure support. Tidal volume. Breathing rate. Inspiratory time. Positive End Expiratory Pressure (PEEP). Pressure and Volume Modes. Have an overall length not to exceed 10". Have an overall width not to exceed 15". Have an overall height not to exceed 12". Two (2) Detachable Battery Packs for Ventilators. Two (2) Roll Stands for Ventilators. All interested firms who can meet the requirements stated above should respond, in writing, INCLUDING ALL OF THE FOLLOWING: Company name. Address. Point of contact (name/title/telephone number/e-mail address). FSS Contract number, if applicable. DUNS number. Tax ID number. Indication of which business category (SDVOSB, VOSB, Small Business or Large Business) for which the organization qualifies. A capability statement that addresses the organizations qualifications and ability to provide the requirement depicted above, and proof (i.e. correspondence from manufacturer) of your firm being an authorized distributor for the manufacturer. Submit responses to the Contracting Officer, Thomas Chesak, at thomas.chesak@va.gov no later than 11:00 a.m. (Central Standard Time), 05 February 2017. VA69D-17-N-0533 Ventilators for VA IHCS shall be referenced on all correspondence regarding this announcement. TELEPHONIC INQUIRIES WILL NOT BE ACCEPTED. The NAICS Code 339113, Surgical Appliance and Supplies Manufacturing, is applicable to this acquisition; the size standard is 750 employees. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract. The Government will use responses to this notice to make an appropriate acquisition decision. This is the only notice, no other copies of this notice will be provided.
