Description
Central Arkansas Veterans Healthcare System Performance Work Statement (PWS) A. GENERAL INFORMATION 1. Title of Project: Transport soiled and clean linen between the Central Arkansas Veteran (CAVHS) campuses in North Little Rock, AR and the Gulf Coast Veterans Healthcare System (GCVHS) in Biloxi, MS. 2. Background: The CAVHS laundry facility is inoperative and pending major renovation. The renovation is anticipated to be completed on/about August 2017. Until the renovation is complete and the CAVHS laundry facility back operational, the CAVHS requires over the road transportation services to transport soiled and clean linens between CAVHS and GCVHS. 3. Scope of Work: The Contractor shall provide labor and equipment required to transport soiled and clean linens between the CAVHS and GCVHS. The government will provide labor to load/unload carts and perform necessary trailer decontamination at each destination. One leg of the trip is approximately 432 miles, estimated driving time 7 hours 25 minutes. The current schedule calls for three deliveries to Biloxi weekly, on Monday, Wednesday, and Friday; however, the government reserves the right to adjust delivery days to Biloxi, based on the government s needs. The government shall provide the carrier 48 hours advance notice of any delivery day schedule changes. The government will contact the contractor to reschedule scheduled runs that fall on a Federal holiday. In the event the government requires additional runs (in addition to the three scheduled weekly runs), the carrier shall be given 24 hours advance notice, and pricing for the additional runs shall be at the agreed to contracted rate. Linen Cart Information: Cart weight: empty = approximately 200 lbs.; dirty full = approximately 500 lbs.: clean full = approximately 480 labs. Cart dimensions: 53 X 26 X55, number of carts per shipment, 33 maximum. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES TASK ONE: Pick-up loads of soiled laundry located at the CAVHS North Little Rock campus. DELIVERABLE ONE: The soiled laundry in bins gets picked-up on the scheduled day/time. TASK TWO: Deliver CAVHS bins of soiled laundry to the GCVHS, Biloxi, MS, by 06:00am, on the schedule d day. DELIVERABLE TWO: The soiled laundry in bins gets delivered to GCVHS by 06:00am on the scheduled day. TASK THREE: Pick-up loads of clean laundry in bins located at the GCBVHS in Biloxi, MS. DELIVERABLE THREE: The clean laundry in bins gets picked-up on the scheduled date/time. TASK FOUR Deliver GCVHS bins of clean laundry to the CAVHS campus in North Little Rock, AR, by 09:00am, on the scheduled date/time. CHANGES TO THE PERFORMANCE WORK STATEMENT The Contracting Officer (CO) is the only individual authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer s Representative (COR) on all matters pertaining to contract administration. The COR is designated to represent the CO in furnishing technical guidance and advice regarding the work being performed under this contract. The foregoing is not to be construed as authorization to interpret or furnish advice and information to the Contractor relative to the financial or legal aspects of the contract. Enforcement of these segments is vested in and is the responsibility of the CO. Only the CO is authorized to make commitments or issue changes affecting price, quantity or performance of this contract. In the event the Contractor effects any such change at the direction of any person other than the CO, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in cost incurred as a result thereof. Invoice and payment information must be submitted electronically to: http://www.fsc.va.gov/einvoice.asp J. GOVERNMENT RESPONSIBILITIES 1. The government shall provide the contractor access to the loading docks where linen carts are to be picked-up/delivered. 2. The government shall provide linen bins. 3. The government shall provide labor to load/unload bins at the CAVHS and GCVHS. The government will secure the bins once loaded inside the trailers. 4. The government shall decontaminate soiled linen trailers prior to loading clean linen at Biloxi. 5. The government shall not move contractor trailers. K. CONTRACTOR EXPERIENCE AND/OR REQUIREMENTS 1. Contract employees shall possess the required CDL to operate transport equipment. Copies of the drivers CDL information shall be included in the contractor s proposal. 2. The contractor shall provide tractor/trailer(s) and required insurance to transport loads. 3. In the event of tractor/trailer failure during transport (anywhere along the transport route), the contractor shall be responsible for acquiring replacement equipment and ensuring the loads are delivered within eight (8) hours of scheduled delivery date/time. Contractor shall include a contingency plan, to cover unscheduled breakdowns, with their proposal. 4. The contractor shall include a copy of the contractor s Federal Motor Carrier Safety Administration Listing and Department of Transportation (DOT) number in their proposal. 5. The contractor shall provide cargo and vehicle insurance coverage documents in their proposal. L. CONFIDENTIALITY AND NONDISCLOSURE 1. The Contractor understands and agrees that confidential information (patients, employee, or facility) must be secured and protected at all times. The Contractor and its personnel shall be held liable in the event a breech of confidentiality occurs as a result of the Contractor or its employees. Each contract employee will be required to read and sign the Contractor Rules of Behavior and provide the original signature page to the COR. M. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS 1. None. Contractor employees will only be hauling clean/soiled linens and will only have access to loading docks at CAVHS and GCVHS. O. EVALUATION FACTORS 1. Contractor s breakdown contingency plan. 2. Contractor s past performance. 3. Price.
