Attachment 2: Invoice and Payment Provisions Attachment 1: Statement of Work Notice Information Solicitation: 17-000999 Agency/Office: National Institutes of Health Location: Clinical Center/Office of Purchasing & Contracts Title: Replacement Whole Blood Centrifuge and Rotor Description(s): This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items", as supplemented with additional information included on this notice. This announcement constitutes the only solicitation; offers are being requested, and a written solicitation document will not be issued. The solicitation number is 17-000999 and is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89. This acquisition will be made pursuant to the authority of FAR 13 to use simplified acquisition procedures for commercial requirements. This procurement is NOT a total small business set aside. The Government intends to issue a Firm Fixed Price type contract. The associated North American Industry Classification System (NAICS) Code is 334516 with a business standard size of 1000 employees. DESCRIPTION OF REQUIREMENT: The Clinical Center has a requirement for a replacement whole blood centrifuge and rotor for the Transfusion Services Laboratory (TSL). Offerors are directed to Attachment 1: Statement of Work (SOW) for all technical requirements and standards of service. DELIVERY ADDRESS: The National Institutes of Health, Building 10, 9000 Rockville Pike, Bethesda, MD 20892. PERIOD OF PERFORMANCE The award is expected to be issued on or around January 23, 2016. PROVISIONS Provisions/clauses may be obtained via www.arnet.gov. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The solicitation does not commit the Government to pay any cost for the preparation and submission of a quote or proposal. It is also advised that the Contracting Officer (CO) is the only individual who can legally commit and obligate the Government to the expenditure of public funds in connection with the proposed acquisition. SAM: Vendors must be registered in the System for Award Management (SAM) prior to the award of a contract. You may register by going to www.sam.gov. You will need the company Dun & Bradstreet number and banking information. The provision at 52.212-2 EVALUATION-COMMERCIAL ITEMS (OCT 2014) is not applicable. The selection will be made via Lowest Price Technically Acceptable (LPTA). Technical acceptability is defined as: A floor model centrifuge for preparing blood components from whole blood donations and a rotor are needed for the Transfusion Services Laboratory (TSL) of the Department of Transfusion Medicine in the Clinical Center, NIH that meets the following salient requirements: 1. TSL requires a minimum capacity to centrifuge 6 blood products at one time. 2. The centrifuge must fit within the space available within the lab (36" X 32" X 40"). 3. The centrifuge must only be designed for centrifugation of whole blood products. 4. The company selling the centrifuge must be able to service and repair the centrifuge - the centrifuge requires biannual preventive maintenance to meet FDA/AABB regulatory requirements. 5. The centrifuge must be listed with the FDA to conform to current Good Manufacturing Practices (cGMP) requirements, by being able to track centrifuge runs with on-board run logging of user, run conditions and error messages. 6. For space considerations, we must have storage of the rotor cover in the centrifuge lid. 7. Per government mandate requiring energy conservation, the centrifuge must have a wind-shielded rotor. 8. The centrifuge meets security needs with user access control and optional password protection. 9. Independently, and not as an agent of the government, vendor shall provide all necessary personnel, supplies, and services to perform the work required for receipt and inside delivery of the centrifuge and rotor 10. The technician shall unpack and deliver the Centrifuge and Rotor to the TSL, placing it in the location where it will be used by TSL staff. 11. The contractors shall supply all parts and supplies needed for the delivery. 12. The contractor includes labor and travel at no additional cost. (d) Comptroller General Examination of Record.
