- Place of Performance Provision Credit Check Authorization Form ACH Vendor Payment Form Business Management Questionnaire Test Method and Test Results for FAB1169 Cover Letter and Solicitation CT2224-17 This is a solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and the solicitation will be available at the Federal Business Opportunities website. The solicitation number is CT2224-17 and this solicitation is issued as a Request for Proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89-1. The North American Industry Classification System code is 313320 fabric coating mills, the SBA small business size standard is 1,000 employees. This requirement is SET-ASIDE 100% for SMALL BUSINESS. UNICOR, Federal Prison Industries, Inc. intends to enter into a one (1) year firm-fixed price Requirements type contract with four (4) one-year options (if exercised) for the following commercial item: Ticking, synthetic fabric vinyl/polyester laminated, weight 12.5 oz per square yard, green color for use in manufacturing mattresses for sale to the GSA and other federal government agencies.
