Description

This Sources Sought/Market Survey Notice is being conducted by the Ann Arbor VA Healthcare System (VAAAHS), 2215 Fuller Road, Ann Arbor, MI 48105 to identify all sources that are capable of providing non-emergent wheelchair van transportation services for eligible beneficiaries of the VAAAHS on a 24 hrs per day, 7 days per week, 365 days per year basis. Trips are generally within a 30 mile radius of the VAAAHS and include transportation between patient pick-up/drop-off locations (patients residences, nursing homes, medical centers, etc.) and the VAAAHS. Contractors must have a sufficient vehicle fleet and driver staffing to safely transport and accommodate non-ambulatory patients and their wheelchairs. If this is a service your business can provide, respond to this Sources Sought Notice by Wednesday, November 23, 2016 at 12:00 P.M. (EST). Responses should include: (1) Business Name and Address (2) Point of Contact Name, Phone Number, and E-mail Address (3) DUNS and NAICS code (4) Business Size SMALL or LARGE (5) Type of Business SERVICE DISABLED VETERAN OWNED, VETERAN OWNED, 8A, HUBZone, WOMEN-OWNED etc. (6) Applicable GSA/FSS Contract Number (if applies). All businesses eligible to provide this service are encouraged to reply and include capability statements as this information may be used to determine potential set-asides for the above noted service.
