SCOPE OF WORK PART 1 PROJECT DESCRIPTION 1.1 Replacement of 4 shower stalls for STA Harbor Beach. PART 2 DESCRIPTION OF WORK 2.1 Remove, dispose and replace 4 existing corner shower stalls. 2.2 Each shower stall is a corner stall 42" x42". 2.3 Remove and dispose of approximately 250 square feet of ceramic wall tile. 2.4 Furnish and install four new 42" x 42" corner shower stalls with acrylic surround. PART 3 LOCATION OF WORK 3.1 Coast Guard Station Harbor Beach 104 Richie Drive Harbor Beach, MI 48441-1042 PART 4 SITE VISIT 4.1 Those who desire access to Station Harbor Beach for bidding or work are to contact DCC Michael Nettles or MK1 Charles Osborne. Once awarded the job, contractors are to arrange time and days for work to be accomplished after receiving notification from the U.S. Coast Guard Contracting Officer to proceed. PART 5 WORK HOURS/COORDINATION 5.1 All communications will go through the POC listed. No work can be modified or changed without their permission. 5.2 Work to be completed should have minimal impact on the unit's operation. In the event any of the utilities need to be secured, these items should be well planned out. 5.3 Normal work hours for the unit is 7:00 AM to 4:00 PM, Monday-Friday, excluding all Federal Holidays. Any work outside these times need to be worked out and approved by the POC ahead of time. PART 6 POINT OF CONTACT 6.1 Point of Contacts: DCC Michael Nettles (313) 568-9474 Michael.O.Nettles@uscg.mil CWO Brian Hall (313) 820-2928 Brian.S.Hall@uscg.mil MK1 Charles Osborne (989) 479-3286 Charles.R.Osborne@uscg.mil PART 7 MILITARY REGULATIONS 7.1 The contractor, his employees and sub contractors shall become familiar with and obey all station regulations. 7.2 All personnel employed on the project shall keep within the limits of the work and avenues of ingress and egress. Personnel shall not enter any other areas outside the site of work unless required to do so in the performance of their duties and have obtained verbal permission from Point of Contact or the Officer of the Day. 7.3 The contractor's equipment shall be conspicuously marked for identification. 7.4 There shall be no smoking in any Coast Guard building and only in designated areas. PART 8 PROJECT COMPLETION / ACCEPTANCE OF WORK 8.1 The contractor will be financially responsible for any and all damages incurred while working on Government property. 8.2 The contractor shall remove and properly dispose of all trash and debris pertaining to the contract work from the limits of government property, as well as adjacent affected areas. 8.3 Disposal of HAZMAT shall be in accordance with Federal, State and local regulations 8.4 All work completed shall be approved by the POC. The site is to be at a minimum as clean of a condition as it was found in. 8.5 Prior to the acceptance of work by the Coast Guard the contractor shall arrange through DCC Michael Nettles a review of work completed to ensure work satisfaction as well as proper clean up. PART 9 LABORER WAGES 9.1 Quote must reflect hourly wages prescribed by the Department of Labor in the attached Wage Determination Document.
