- Sources Sought Notice This is a Sources Sought Notice only. This does not constitute a solicitation. The information gathered from this request for information (RFI) is for planning purposes only. The Government is not obligated, nor shall it pay for, any information received from potential sources as a result of this Sources Sought Notice. Network Contracting Office 8 (NCO 8) is currently conducting market research to identify Small Businesses capable of providing full service maintenance on two (2) 3M Steri-Vac 8XLP Sterilizers located at the C.W. Bill Young Veteran Affairs (VA) Medical Center, 10000 Bay Pines Blvd, Bay Pines, Florida 33744 in accordance with the below Statement of Work (SOW). The resultant contact shall be for a one (1) year base year (12/1/2016 11/30/2017) and four (4) option years. Any Small Business interested in participating in any future solicitation is encouraged to review the requirements and respond with confirmation that they can provide a quote for this requirement. Statement of Work 1. SCOPE OF WORK: Contractor shall provide full service maintenance on two (2) 3M Steri-Vac 8XLP Sterilizers, which includes all scheduled preventive maintenance (semi-annual), unscheduled repairs, OEM repair parts, calibrations, hardware, software/software updates (as needed), and phone support for the following equipment: 3M Healthcare Steri-Vac 8XLP, 108187, SR #351303, STERILIZING UNIT: ETHYLENE OXIDE 3M Healthcare Steri-Vac 8XLP, 108188, SR #351302, STERILIZING UNIT: ETHYLENE OXIDE Contractor shall have full access to the original manufacturer s software for calibration and repairs as required. No subcontracting of any portion of this service is authorized. All technicians providing service on this equipment shall be factory trained by the original manufacturer of the equipment. Documentation of this training needs to be available upon request. 2. PERIOD OF PERFORMANCE: December 1, 2016 through November 30, 2017 with four (4) option years. 3. LOCATION OF WORK: C.W. Bill Young Veteran Affairs (VA) Medical Center (CWBYVAMC), 10000 Bay Pines Blvd, Bay Pines, Florida 33744, Building 100 Room #1C109. 4. UNSCHEDULED REPAIRS: The contractor s field service engineer will respond by phone within 2 hours and respond on site within 48 hours to correct the problem. Uptime requirements, excluding scheduled preventive maintenance, will be 97% of the normal working hours and working days during the term of the contract. 5. SCHEDULING ARRANGEMENTS: Except by special alternative arrangement, scheduled maintenance shall be provided only during normal working hours (8:00 a.m. to 5:00 p.m.), Monday through Friday, excluding Federal Holidays (New Year s Day, Martin Luther King Day, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day). All work done outside the normal coverage time should be first approved through the Biomedical Engineering Section, Bay Pines. The contractor will always contact the Biomedical Engineering Section Room# 1F122-100, (727) 398-6661 extension 15601 (after hours, extension 17777) before and after arrival at the site. JUSTIFICATION: In accordance with the manufacturer s requirements, NFPA, the Joint Commission, and local policy, scheduled and unscheduled maintenance is required on this equipment. 7. PERSONNEL CLEARANCES AND PRIVACY CONSIDERATIONS: The Contractor will ensure that their personnel and subcontractor personnel meet the privacy standards as set forth by the Health Insurance Portability and Accountability Act of 1996 with respect personal and confidential information that they may come upon, while servicing medical equipment. 8. DOCUMENTATION: a. Contractor shall provide the Biomed COR with individual written reports which describe the maintenance and repair service performed on the equipment in sufficient detail so as to be acceptable by field inspectors of the Joint Commission of Accreditation of Healthcare Organizations. This shall include a list of all parts replaced, all service performed as well as a statement that the equipment is operating per manufacturer s specifications after repair. The service report shall be signed by the contractor s service technician and by a designated CWBYVAMC Biomedical Engineer, with a copy given to CWBYVAMC Biomedical Engineering Service Office for their internal records. b. Prior to award, offeror shall have a facility to include personnel, test equipment, parts inventory, training certificates, licenses and technical documentation available for inspection by VA Medical Center personnel and be able to show written evidence to technical qualifications of personnel, test equipment (and calibration documentation) licenses and availability of parts. Parts availability shall be written documentation from either the equipment manufacturer or authorized parts supplier. In either case, offeror shall be able to show the availability of parts to their company within 24 hours after initial call. For contracts for maintenance and repair services from other than the original manufacturer or a designated representative, the Contractor shall be fully responsible for obtaining all technical documentation necessary to fulfill contractual obligations. c. Contractor shall warrant the services performed under this contract to ensure performance is provided in a good workmanship manner and conforms to the standards of the industry. This warranty shall be expressly given and in place of all other warranties, expressed or implied, statutory or otherwise, and shall be the only warranty given by the Contractor. 9. INCIDENT OF NEGLIGENCE: When services and parts are required as a result of accident, abuse, misuse or negligence by other than the Contractor or his representative, and are not included as a part of PM inspection or service calls, such needs shall be reported to the Contracting Officer s representative along with estimates of charges. The Contracting Officer and Representative will evaluate these needs and authorize appropriate action, which may include the need to modify the contract to add additional repairs. The parts shall be installed under the terms of the contact and each invoice is to include a legible itemized list of labor charges and parts costs. The Government will incur no charges for service and parts due to accident, abuse, misuse or negligence by the contractor or his representative. 10. END OF PERFORMANCE PERIOD: Contractor guarantees that all equipment covered in this contract shall meet or exceed manufacturer s performance specifications at the contract expiration date. At the end of the contract term (to include exercised options) there will be a final inspection of all equipment at which the Contractor and the Contracting Officer s Representative will be present. Any deficiencies noted will be the responsibility of the current Contractor and shall be corrected by the terms of this contract.
