Description
STATEMENT OF WORK 1. BACKGROUND: The Minneapolis VA Healthcare System (MVAHCS) provides dental prosthodontics to veterans receiving dental care from the Minneapolis VA Health Care System (MVAHCS) and from VA NW Metro Community Based Outpatient Clinic (CBOC) in Ramsey, MN. The CBOC Dental Service Clinic is under the jurisdiction of the Minneapolis MVAHCS and is included in this contract. Eligible veteran beneficiaries of MVAHCS Dental Services cover a broad range of ages and needs. The MVAHCS averages 23,000 dental patient visits per year. 2. SCOPE: The MVAHCS has a requirement for contractor to provide fixed and removable prosthodontics and dental appliances for veterans receiving dental services from the Minneapolis VA Medical Center and from the NW Metro CBOC Dental Clinics providing dental care to veterans. All prosthetics and dental items shall be provided per the list of products and prices in the Schedule of Items and in accordance with the requirements as specified herein. 3. SPECIFICATIONS/STATEMENT OF WORK: (a) General Requirement: The contractor shall provide fixed and removable prosthodontics, dental appliances and supplies. All products are to be fabricated at the contractor s facility unless otherwise approved by the MVAHCS Dental Service. Contractor will use only high quality American Dental Association (ADA) certified raw materials in the fabrication of dental products. All products supplied will be in accordance with the terms and conditions of this statement of work. (b) Pick-up and Delivery: Contractor s Laboratory must be able to facilitate daily pick-up and delivery services at the MVAHCS Dental Service sites. All items such as impressions, teeth, prescriptions and finished products will be picked up daily from the MVAHCS Dental Service Clinic and the VA NW Metro Community Based Outpatient Clinic (VA NW CBOC) and delivered back to the appropriate VA Dental Clinic per required turn-around times. MVAHCS Dental Service locations are: VA Dental Service, Room 2U-121 Minneapolis VA Healthcare System One Veterans Drive Minneapolis MN VA NW Metro CBOC Dental Service 7545 Veterans Drive Ramsey MN Pick-up and delivery services include the following: Contractor will provide a minimum of one daily pick-up and delivery to the MVAHCS Dental Service Clinic and VA MW Metro CBOC, Monday through Friday, excluding Federal Holidays, between the hours of 7:00am and 3:30 pm. There are currently 10 holidays observed by the Federal Government, they are: New Year s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, and Christmas. There may be other days during a year that are declared by the President of the United States to be a national holiday. Pick-up and delivery services are required to ensure that casework arrives on time and safely. Contractor will supply proper case carriers for transport. These services will be provided at no additional cost to the VA. The Contractor s Dental Lab will provide detailed tracking system for proper identification of dental prosthesis. Electronic Submission: Contractor s Dental Laboratory must have capability of electronic submission of cases for example, including but not limited to: CAD/CAM and digital scanning technologies. If vendor is proposing daily pickup and delivery via mail services, they will need to make arrangements for pickup and delivery to the dental suite, as well as provide documentation detailing how they will meet the required turnaround times. All pricing will be inclusive of shipping. (c) Fixed Dental Supply Requirements: Contractor shall provide the following fixed dental laboratory items to the MVAHCS and CBOC: Gold precious and semi-precious crowns, bridges, inlays, and onlays (full and partial fixed gold prosthesis) Ceramic crowns and bridges (including but not limited to all Zirconia and Zirconia supported fixed prosthesis, pressed Emax monolithic crowns for posterior teeth and Emax layered for anterior and esthetically demanding restorations). Cast post and cores Porcelain fused to metal crowns and bridges Model, Die and Articulate Temporary crowns Diagnostic wax-up Implant abutments and crowns Maryland bridges abutment wings Precision attachments Porcelain repair Acrylic trays Bleaching trays Thermoplastic mouth guards (e.g. Bruxese) Acrylic night guards (e.g. hard acrylic full occlusal coverage orthotics) Acrylic resin custom temporary crowns and fixed partial dentures Trinia fixed prostheses Custom Shade Matches at MVAHCS and CBOC dental clinics Essix retainer prosthesis. Clear temporary laminate duplicate of a wax-up for a full mouth reconstruction Surgical stents and guides (milled, printed, or custom fabrication) using digital scans as well as patient casts Survey crowns, bridges, and fixed prosthesis Verification jigs Porcelain butt margins Turn Around Times Fixed Dental Supply: Turnaround time required for each fabrication or repair from receipt at contractor s lab shall be within the time frame listed below. In case of longer times, the Minneapolis VA s Point of Contact (POC) must be made aware of the delay before the maximum number of days has expired. All crown and bridge procedures and cast frame works shall be completed within ten (10) working days from date of pick-up from the VA to date of delivery to VA. (d) Removable Dental Supply Requirements: Contractor shall provide the following removable dental laboratory products: Complete dentures (including custom trays, bite rims, set-up, processes & finish) Implant Overdentures (including custom trays, bite rims, set-up, processes & finish) Gold bars and attachments for Overdentures Fixed detachable frameworks and denture teeth Cast partial dentures (including frameworks, set-ups, process and finish) All acrylic partial dentures (including those which require wrought wire clasps, with set-up, process & finish) Complete and partial denture repair and strengthener Compete and partial denture relines and rebases Obturators, Palatal augmentation prosthesis, Palatal Lifts (including custom trays, bite rims, set-up, processes & finish) Trinia removable prostheses Diagnostic Wax-up with denture teeth TCS partial and complete dentures (including custom trays, bite rims, set-up, processes & finish) Denture soft liner Denture repair to teeth, base, extensions, metal frame, and wire clasps Fiber reinforcement and strengthener Immediate Denture Conversion for immediate loaded implants Please Note: The MVAHCS may supply teeth to the contractor for dentures. Turn Around Times Removable Dental Supply: Turn-around time required for each fabrication or repair from receipt at contractor s dental lab shall be within the time fame listed below. In case of longer times, the Minneapolis VA s POC must be made aware of the delay before the maximum number of days has expired. Denture Steps & Trays: All complete and partial dentures steps shall be completed within five (5) working days from date of pick-up from the VA to date of delivery to VA. - EXCEPT - Repair/Rebase/Reline: Shall be completed with one (1) working day from date of pick-up from the VA to date of delivery to VA. Removable Partial Denture Frameworks which shall be completed within ten (10) working days from date picked up at the VA to date of delivery to VA. (e) Certifications: The contractor shall use only high quality American Dental Association (ADA) certified raw materials in the fabrication of all dental products provided. At the time of delivery, contractor is required to provide documentation as to the type of metal and other materials used in fabricating the dental product. Contractor is required to certify that all work is completed in the United States and made with only ADA certified materials. The Dental Laboratory providing services to the VA must be a Certified Dental Laboratory (CDL) as certified by the National Board for Certification (NBC) in Dental Laboratory Technology and as recognized by the American Dental Association (ADA) and the American College of Prosthodontists (ACP). (f) Quality Assurance: All products provided to the VA must be of the same consistent high quality as would be offered to the general commercial public in the United States. Contractor will use only high quality raw materials purchased from ISO 9001/CE registered companies. The MVAHCS will provide practitioner specific preferences to the contractor s dental Lab. Contractor shall have a quality assurance process (QA) that will guarantee a product that meets the practitioner s prescription and that meets the patient s natural esthetics and provides superior fit with minimal chair time. Laboratory will use a computerized shade verification system as well as perform internal technical audits to ensure consistent high quality and timely restorations. Contractor s dental laboratory must meet all regulations imposed on dental laboratories in regards to safety, cleanliness, and quality assurance standards as set forth by Occupational Safety and Health Administration (OSHA), U.S. Food and Drug Administration (FDA), and state agencies. As required for certification by the NBC, the contractor must have quality assurance standards and procedures in place to monitor the fabrication process of prosthodontic products to ensure quality. QA procedures include, but are not limited to, production processes, re-make circumstances, physical facility space and equipment cleanliness and maintenance, infection control standards, and the education and training of employees. The dental laboratory must employ Certified Dental Technicians (CDT) with qualifications and experience in the fabricating of: complete dentures, partial dentures, crown and bridge, ceramics and orthodontics. (g) Warranty: Contractor warranties all products against defects in material and workmanship. The contractor will be responsible for remaking all dental products that are defective or do not fit due to contractor s error. If a crown does not fit the die, it will be considered the fault of the contractor and the crown will be remade at no additional expense to the Government. If the crown fits the die, but not the patient s mouth, it will be considered the fault of the Government, and the remake will be at the Government s expense. If, in the case of porcelain work, the porcelain shatters within twelve months of fabrication, it will be considered the fault of the contractor and will be remade at no additional expense to the Government. If a partial denture framework does not fit the master cast, it will be considered the fault of the contractor and the framework remade at no additional expense to the Government. If the framework fits the die, but not the patient s mouth, it will be considered the fault of the Government, and the remake will be at the Government s expense. (h) Rework: The contractor will provide items that meet the practitioner s prescription and the esthetic and functional requirements of the patient. The contractor will, at no additional expense to the VA, remake all dental items that do not meet the prescription or are flawed due to contractor error. For example, if a partial denture framework does not fit the master cast, it will be considered flawed and unacceptable. The framework will be remade at no additional expense to the VA. Changes to the partial denture framework design not approved by the dentist on the laboratory prescription will be considered flawed and unacceptable and will be remade at no additional cost to the VA. (i) Exchange of Inventory: If the MVAHCS does not have a particular color or size of tooth in inventory, the contractor shall agree to exchange the required color/size tooth in their inventory with VA inventory of like kind. Contractor shall determine what inventory is considered of same value for exchange. (j) Customer Support: Contractor s Dental Laboratory will provide: professional case consultation services which may require either on-site or tele-dentistry services; responsive and knowledgeable customer support representatives; computerized tracking; and personalized pick-up and delivery services. (k) All-Inclusive Pricing: The contractor will bill at an all-inclusive rate. Contract pricing must include all materials, supplies, labor, packaging, transportation and customer service costs. Partial billing or split billing for set-up steps and finish of dentures is not acceptable. (l) Offeror Documentation: The offeror shall provide documentation with their quote demonstrating their ability to meet the requirements in the statement of work.
