Description
The purpose of this Sources Sought Notice is to search for small business concerns capable of providing Air Freshener services at the Coatesville VA Medical Center.   This Sources Sought Notice is for market research purposes only and shall not be considered an Invitation for Bids, Request for Quotations, or a Request for Proposal. Do not submit a proposal or quote. This market research is issued for information and planning purposes only and does not constitute a solicitation nor does it restrict the Government as to the ultimate acquisition approach. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Additionally, there is no obligation on the part of the Government to acquire any products or services described in this sources sought. You will not be entitled to payment for direct or indirect costs that you incur in responding to this Sources Sought Notice. Any contract that might be awarded based on information received or derived from this market research will be the outcome of a competitive process. Interested parties are responsible for adequately marking proprietary, restricted or competition sensitive information contained in their response. The Government does not intend to pay for the information submitted in response to this Sources Sought Notice. The draft Statement of Work (SOW) follows (and attached): =================================================================== DRAFT STATEMENT OF WORK BELOW (and attached) =================================================================== Statement of Work (SOW) On Site Air Freshener Installation and Monthly Maintenance Coatesville VA Medical Center 1400 Black Horse Hill Road Coatesville, PA 19320-2096 Summary:   This contract is for services to place air freshener dispensers and routine maintenance in identified restrooms throughout the Department of Veterans Affairs (VA) Coatesville Medical Center.   Scope of Work: The Department of Veterans Affairs (VA) Coatesville Medical Center intends to use the services of a contractor to supply, clean and replenish air fresheners and bowl drips throughout the Coatesville VA Medical Center on a monthly basis Required Work: The contractor shall provide all labor and equipment and shall be responsible for cleaning and replenishing all air fresheners and bowl drips throughout the medical center. Replacement documentation /certification must be provided. (I.e. once service is completed documentation verifying locations provided). Contractor shall maintain liability insurance throughout the term of the contract following all pertinent federal and state regulations with regard to cleaning and maintaining dispensers in a medical center facility. All equipment must comply with applicable safety standards, including OSHA. While on VAMC premises, contractor employees will wear a company uniform and a visible identification badge which contains a photograph of the employee. All contractor vehicles and collection bins will be clearly identified with the company s name. The contractor will provide the services identified in this contract between the hours of 7:00 AM and 3:00 PM, Monday of each week, except when a legal holiday falls on said day, in which case service will be provided Tuesday. The contractor will provide at least 48 hours notice if for some unforeseen circumstance they will be unable to keep the scheduled date for service. The contractor will be responsible to ensure contractor employees providing work on this contract are fully trained and completely competent to perform the required work.The contractor is also responsible for completing background investigations on all personnel who will be on site. The results of the background investigations must be furnished upon request and be available to the Chief of Information Security Officer; maintained by COR. Equipment: The contractor shall provide mobile equipment to do dispenser maintenance Quantities: The medical center reserves the right to increase or decrease the quantity of material. As they may vary from listing initiated through vendor verification walkthrough. Additional Requirements: National Holidays: The ten holidays observed by the Federal Government are: New Year s Day Martin Luther King s Birthday Memorial Day Independence Day Labor Day Columbus Day Veterans Day Washington s Birthday Thanksgiving Day Christmas And any other holiday specifically declared by the President of the United States to be a national holiday. HIPAA: The contractor will have to complete a standard Business Associate Agreement (BAA) in accordance with the Health Insurance Portability and Accountability act of 1996 (HIPAA) after award. The contractor will also provide the VAMC with copies of any and all applicable Federal, State of Pennsylvania, Chester County licenses of any sort necessary to provide the required services to the VAMC. =================================================================== DRAFT STATEMENT OF WORK ABOVE (and attached) =================================================================== Any interested small business concern must submit a no longer that 3 page capability statement addressing its ability to meet the requirements listed above to Mark Murray, Contract Specialist, at mark.murray4@va.gov no later than Wednesday, 26 October 2016, 12:00 PM. Interested parties are also welcome to identify any ambiguities with the SOW and are welcome to suggest enhancements to the SOW and include them separately with their capability statement. Questions regarding this Sources Sought Notice will not be answered.
