Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. Solicitation number W9114F-16-T-0093 is issued as a request for quotation (RFQ). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50, effective 16 March 2011. Not a set-aside. North American Industry Classification System (NAICS) code for this solicitation is 621910. The Regional Health Contracting Office- Europe has a requirement to procure non-personal Ambulance Services during DoDDS interscholastic contact sports events within the AFNORTH Schinnen Netherlands Facilities geographical areas of responsibility. Performance will be for a Base Period of 11 months and four (4) Option Periods of 12 months each with a start date of 15 November 2016. The contract line items (CLINs) for this acquisition are as follows: Line Items 0001 + 1001 + 2001 + 3001 + 4001. QTY 80 EACH (The unit of measure "Each" is defined as a 60-minute period, or the fraction thereof). Non-personal Ambulance Services during High School Sporting Events such as football, basketball, wrestling, soccer, cheerleaders, etc.). The Contractor shall furnish all personnel, equipment, vehicles, supplies, management and supervision necessary to provide/coordinate emergency medical service during DoDDS interscholastic contact sports events within the AFNORTH Schinnen Healthcare Facilities geographical areas of responsibility. The Contractor shall provide, at a minimum, a properly equipped ambulance vehicle, an ambulance driver and a trained/qualified Emergency Medical Technician, EMT I-85, or equivalent in the Netherlands as described in the Uniform Glossary of Ambulance Care terms based on the perusal Landelijk Protocol Ambulancezorg (LPA) 8.1dated June 2016), for the duration of all scheduled contact sports events. All services provided by the Contractor shall be covered by the terms and conditions of this contract, performed in accordance with the Performance Work Statement (PWS), all applicable United States (U.S.) and local laws, regulations, standards, provisions and directives. The Technical Factor has three (3) sub-factors. Quoters must explain how they will comply with the requirements in the RFQ by submitting written technical quotes which describe: Subfactor 1: TRAINING AND EXPERIENCE. Quoters shall submit written proof/certificate of: a) properly equipped ambulance vehicle; b) of the ambulance driver; and c) of a trained/qualified Emergency Medical Technician, EMT I-85, or equivalent in the Netherlands as described in the Uniform Glossary of Ambulance Care terms based on the perusal Landelijk Protocol Ambulancezorg (LPA) 8.1dated June 2016). Quoters shall submit written proof/certificate of the Emergency Medical Technician qualifications and capabilities, and minimum two (2) years in autonomous performance of medical assistance services. Subfactor 2: TECHNICAL APPROACH. Quoter's quote shall clearly describe what resources - personnel, vehicles, equipment, tools, materials, supplies, training and gear - will be dedicated to meeting the requirements outlined in the PWS. Subfactor 3: ENGLISH PROFICIENCY. Quoter's personnel shall be able to read, and communicate in the English language. Quoter's personnel shall be able to compile required reports; to compile and complete required forms. Not all personnel in service at the same time, must be able to communicate in English, but at least one per every crew. FACTOR II: PAST PERFORMANCE. Quoters shall submit a list of no less than three (3) and no more than five (5) contracts or sub-contracts performed during the past five (5) years and must have been of a similar size and scope of the contract that will be awarded as a result of this solicitation. Contracts listed may include those entered into with the U.S. Government, local government, and commercial customers. Quoters shall provide references from hospitals or other related services. Quoters will provide the names of persons for each contract, their telephone number s and e-mail addresses in order for the Government to contact them regarding Quoter's past performance. The Government may contact independent sources not provided by the Quoter. FACTOR III: PRICE. Quoters shall submit prices on the price schedule in the RFQ. Price will be evaluated for reasonableness based on anticipated adequate price competition. Price analysis will be performed through the use of comparative price quotation analysis for technically acceptable quotations. Price will not be rated. The extended amount for each contract line item will be added up for a grand total and will be used to calculate the Quoter's total price for the Base Period and all Option Periods. Contract award will be made to the technically acceptable offer with the lowest acceptable total five-year price. All CLINs shall be priced. If you fail to price all CLINs, your offer will not be considered for award. Para (n) ELECTRONIC QUOTES. Vendors may submit electronic quotes in response to this RFQ. The quote must arrive by the time specified on Page 1, Block 8 of the Standard Form 1449 (RFQ )to the following email address: domenico.maddaloni.ln@mail.mil Vendors may call Domenico Maddaloni at 011-49-6371-9464-5414 to verify receipt of the quote. If the vendor chooses to email the quote, the Government will not be responsible for any failure of transmission or receipt of the quote, or any failure of the vendor to verify receipt of the emailed quote. If receipt of the quote submitted by email is not verified within 24 hours of submission, the vendor must resubmit their quote by fax. Para (o) QUESTIONS. Prospective vendors are strongly encouraged to submit any questions regarding this RFQ no later than the 19 October 2016 to domenico.maddaloni.ln@mail.mil or by fax (011) 49-6371-86-8070. If the vendor chooses to email the questions, the Government will not be responsible for any failure of transmission or receipt of the questions, or any failure of the vendor to verify receipt of the emailed inquiry. If receipt of questions submitted by email is not verified within 24 hours of submission, the vendor must resubmit their inquiry by fax. ACQUISITION PROCEDURE. This is a commercial item under FAR Part 12 and the acquisition procedures to be used for this purchase will be in accordance with FAR Part 13.5. SPECIAL NOTE: All vendors must be registered in the System for Award Management (SAM) prior to award. Lack of registration shall make a quote ineligible for award. Vendors may obtain information on registration and annual confirmation requirements by calling 1-866-606-8220 or via Internet at http://www.sam.gov. All quotes are due on the date and Central European Time (CET) time shown on page 1, Block 8 of the SF 1449. Quotes may be sent via email to domenico.maddaloni.ln@mail.mil, or faxed to 011-49-6371-86-8070. If the Quoter does not receive confirmation that the quote was received within 24 hours of submission, the Quoter shall fax a copy of the quote to 011-49-6371-86-8070. Award will be made using the lowest price technically acceptable (LPTA) evaluation process. Award will be made to the responsible Quoter on the basis of the lowest evaluated price of quotes meeting or exceeding the acceptability standards for technical factors. The Government intends to make an award without conducting discussions. After the initial evaluation of quotations is completed, if one or more quotations are rated Acceptable Technical, the initial evaluation will become the final evaluation of quotations and award will be made without discussions to the lowest priced quote. If any one (1) technical subfactor is rated Unacceptable after the initial evaluation, the technical factor will be rated Unacceptable and the quote will no longer be considered for award. If all quotations submitted result in an unacceptable technical evaluation, then clarifications may be necessary. In that event, the Government evaluation team will evaluate the quoter's response to the Government exchanges with each Quoter. FACTOR I - TECHNICAL EVALUATION STANDARDS: The Quoter's technical quote will be evaluated using the following standards for a final rating: 1. Acceptable - A quote that meets the Government's technical requirements identified in the Request for Quotation. 2. Unacceptable - Quote A quote that fails to meet the Government's technical requirements identified in the Request for Quotation.. FACTOR II- PAST PERFORMANCE STANDARDS: The Government will use the following standards to rate Past Performance: 1. Acceptable - Based on the Quoter's performance record, the Government has a reasonable expectation that the Quoter will successfully perform the required effort, or the Quoter's performance record is unknown. (See note below.) 2. Unacceptable - Based on the Quoter's performance record, the Government has no reasonable expectation that the Quoter will be able to successfully perform the required effort. (See note below.) NOTE: In the case of a Quoter without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the Quoter may not be evaluated favorably or unfavorably on past performance. Therefore, the Quoter shall be determined to have unknown past performance. In the context of acceptability/unacceptability, unknown shall be considered acceptable. FACTOR III: PRICE. The Government will select the quote that is based upon the lowest evaluated price, technically acceptable quote. All Contract Line Item Numbers (CLINS) must be priced by the quoter. If the quoter failed to price all CLINs, the quote will not be considered for award. The extended amount for each contract line item will be added up for a grand total and will be used to calculate the Quoter's total price for the Base Period and all Option Periods. The Government may determine that a quote is Unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (a) Options. The Government will evaluate quotes for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that a quote is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (b) A quotation is not an offer and, consequently, cannot be accepted by the Government to form a binding contract. Therefore, issuance by the Government of an order in response to a supplier's quotation does not establish a contract. The order is an offer by the Government to the supplier to buy certain supplies or services upon specified terms and conditions. A contract is established when the supplier accepts the offer. (c) The word "quoter" shall be substituted for the word "offeror" wherever it appears in FAR 52.212-3, Alternate I and DFARS 252.212-7000.
