Description
This is a sources sought to determine the availability of potential sources having the capabilities to provide the SPECIFIED REQUIREMENT. All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a sources sought synopsis only. Questions should be submitted by email to eric.winger@va.gov. Provide only the requested information below. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, FSS/GSA contract schedule holders or large business) relative to NAICS 325412, Pharmaceutical preparation manufacturing. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on the FBO website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. SPECIFIED REQUIREMENT: Network Contracting Office 10, Marion, IN is seeking sources for a potential contractor to supply the following items for the NORTHERN INDIANA HEALTHCARE SYSTEM VETERANS AFFAIRS MEDICAL CENTER IN FORT WAYNE, IN: ITEM NO. DESCRIPTION QUANTITY UNIT See statement of work and estimated quantities below. Include past experience in providing this items to the VA, other Government (Federal or State) agency, or for a private facility. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. Your response should include both the STATEMENT OF CABABILITY and BUSINESS SIZE AND SOCIO-ECONOMIC STATUS information as explained above. Please place "Attention: Radiopharmaceuticals for Fort Wayne" in the subject line of your email. This notice is to assist the VA in determining sources only. A SOLICITATION IS NOT CURRENTLY AVAILABLE. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Statement of Work (SOW) for Radiopharmaceuticals, VA Northern Indiana Health Care System (VANIHCS) PART I: GENERAL INFORMATION A. Introduction VANIHCS, Fort Wayne, IN is contracting for a centrally located Nuclear Pharmacy to provide both diagnostic and therapeutic unit dose radiopharmaceuticals. B. Scope The Contractor shall supply the Imaging Service, Nuclear Medicine Section with the radiopharmaceuticals listed on the Department of Veterans Affairs Materials Permit No 13-11983-01, Amendment No. 24 for Fort Wayne VAMC. The Contractor must be capable of supplying Nuclear Medicine with the required unit doses and cold pack approved for nuclear medicine use as needed by VA. PART II: WORK REQUIREMENTS A.Technical Requirements The Contractor shall accept daily orders that shall be calibrated for the time requested by VA. The Contractor shall deliver the material at least twice a day, the first drop off occurring by 7:30 a.m. from orders placed the evening before. The second drop off will be scheduled as needed by the Nuclear Medicine technicians. Weekend and emergency deliveries are also required. The contractor shall deliver the materials in accordance with the shipping protocol of the Nuclear Regulatory Commission and the State of Indiana and Department of Transportation (DOT) rules and regulations. These standards include that the delivered material shall be contained in a lead pig and locked in an appropriate external container. All packaging of radioactive material shall be clearly labeled RADIOACTIVE on the outside of the shipping carton. All appropriate package signage will be observed according to DOT and Indiana rules and regulations, as usual and customary. Contractor shall meet all Nuclear Regulatory Commission, Department of Transportation, FDA, OSHA and all other agency rules and regulations (Federal and State). Contractors shall perform all quality control procedures as required by Federal and State regulatory agencies. Deliverables The pharmaceuticals need to be contained in a syringe labeled with the description and dosage of what was ordered to eliminate possible administration errors. A delivery is required Monday through Friday excluding Federal holidays as requested. The Contractor shall include provision for return of, and credit for, cancelled doses. The contactor shall also provide for government use of software and equipment (hot lab monitoring software) for the term of this agreement specifically for the monitoring and managing of the radiopharmaceutical products provided under this contract. The software and equipment will be used to maintain accurate radiation safety records for regulatory review, as well as to provide inventory tracking and waste management monitoring. PART III: SUPPORTING INFORMATION A. Security No deliveries of unit doses will be accepted except by the courier of the supplier. All couriers shall wear picture ID that is clearly visible while making deliveries of radiopharmaceuticals. B. Place of Performance VANIHCS, 2121 Lake Avenue, Fort Wayne, IN 46805 C. Period of Performance The performance period is from date of award through September 30, 2017 with four one-year option renewals. Date of award is anticipated to be October 1, 2016. D.
